The Prevalence or the Intensity of Dentine Hypersensitivity/Root Sensitivity Following Periodontal Therapy-A Systemic Review

牙本質(牙根)知覺過敏症在牙周病治療後的盛行率或敏感程度─系統性文獻回顧

摘要


背景:牙本質知覺過敏症或牙根敏感症在牙科臨床上是一種相當常見的症狀,影響不同年齡的人。然而,以往文獻關於牙本質(牙根)知覺過敏症在牙周病治療後(包含非手術和手術部分)的盛行率或敏感程度的調查,卻很鮮少有這方面的資料。目標:系統性回顧並整理牙本質(牙根)知覺過敏在牙周病治療後的盛行率或敏感程度。材料與方法:使用手搜尋(hand searching)和電子資料庫(PUBMED),針對西元2011年4月30 日所發表過文章進行篩選。結果:從891篇文章中,挑出12篇文章作分析。由於個別文章異質性高,因此並未採納統合性分析。結果顯示牙本質知覺過敏症的盛行率在非手術治療一天後約為62.5%~90%,一週之後約為52.6%~55%。在手術治療一天後盛行率約為76.8%~80 .4%,一週後約為36.8%,二週後約為33.4%,四週後約為29.6%,八週後約為2 l.7 %。此外,牙本質(牙根)知覺過敏症在牙周病治療後的敏感程度則為輕度至中度不等的短暫疼痛。結論:病人在接受牙周病治療後很容易患有牙本質(牙根)知覺過敏症,儘管其盛行率和敏感程度會根據治療方法與療程長短不同而有所變化。(台灣牙周醫誌16: 215-234, 2011)

Background:dentine hypersensitivity (DH) or root sensitivity (RS) is a well known clinical condition that is reported to affect the adult population at various stages of their life. However, there are limited data available in terms of prevalence and intensity of DH/RS following periodontal therapy (such as nonsurgical and surgical procedures)Objective:To review the available published literature systemically in order to update the reported prevalence or the intensity of DH/RS following periodontal therapy.Materials & Methods: The search strategy included using hand searching and electronic databases (PUBMED) up to 30^th April 2011.Results: 12 studies were selected from the relevant 891 papers and the statistical analysis was not conducted in the present review due to the heterogeneous nature of the included studies. The outcome indicated that the reported prevalence for DH/RS following nonsurgical intervention was between 62.5%~90% in treatments after one day which subsequently decreased to approximately 52.6%- 55% after one week. The prevalence of DH/RS following surgical therapy ranged from 76.8%~ 80.4% one day after treatment, and subsequently decreased over time 36.8% after one week, 33.4% after two weeks, 29.6% after four weeks and 21.7% after eight weeks. In this review, DH/RS appears to be relatively mild/moderate in nature and transient in durationConclusions: It is evident that from reviewing the 12 included studies that patients may suffer some mild discomfort from DH/RS following periodontal procedures although both the prevalence and intensity of DH/RS may vary depending on the duration and the type of procedure involved.
