一位曾有母乳哺餵失敗經歷之剖婦產經產婦的護理經驗
The Nursing Process on a Woman with Nonsuccessful Postpartum Breastfeeding Experience after Caesarean Section
摘要
本篇個案報告是在描述一位29歲的剖腹產經產婦，曾有母乳哺餵失敗經驗的護理過程。照護期間自2009年9月29日至2009年10月2日；以Gordon十一項功能性評估作爲引導，經由直接護理個案方式、身體評估、會談及觀察等方式蒐集資料，確立三項護理問題：(1)低效性母乳哺餵／與曾有母乳哺餵的失敗經驗有關；(2)急性疼痛／與剖腹生產之傷口、經產婦之子宮收縮有關；(3)疲憊／與身體狀況不佳有關。經由此次護理經驗，體認出適時給予個案衛教之重要性，護理人員經由護理評估運用各種適切的護理措施，協助產婦有效的哺餵母乳、緩解疼痛及減輕疲憊感，以達成產後之生理、心理及社會之適應。

This article described a nursing experience for a 29-year-old multipara underwent nonsuccessful experience of breast-feeding after Caesarean section. The period of this case started from September 29th 2009 to October 2nd 2009. With the Gordon's 11-item functional health pattern assessment tool, nursing problems were identified through direct patient care, physical assessment, case interview and observation. Major nursing problem were identified, including: (1) inefficiency of breast-feeding is related to nonsuccessful experience of breast-feeding. (2) Acute pain is related to the Caesarean section of the wound and the contraction of uterine. (3) Fatigue is related to poor health. From this nursing experience, the importance of correct health education and appropriate assistance in cases of need is recognized. Nursing people by using a variety of measures would help promotion of breastfeeding, maternal and effective pain relief in order to facilitate its postnatal physiological and psychological adjustment.
