提升外科病房住院病人藥物指導率

用藥疏失會產生嚴重傷害或導致病人死亡，醫療專業人員應該提供病人安全用藥，以提升藥物效率和疾病的恢復。本專案旨在提升外科病房住院病人藥物指導率，調查其主要原因為護理人員對藥物認知不足、缺乏在職教育、無藥物指導單、未制定住院病人藥物指導作業標單及缺乏讀物指導稽核機制。經專案小組加強人員教育訓練、與資訊室及藥劑科共同設計藥物指導單、制定住院病人藥物指導之作業標準及外科病房住院病人藥物指導率之稽核機制。經介入方案實施後，外科病房住院病人藥物指導率自72.6%提升至94.0%，護理人員藥物認知自73.3分提升至96.7分，調查住院病人對藥物作用、副作用及注意事項之說明的滿意度84.4%提升至91.4%。顯示本專案達到專案目的，並證實跨領域團隊合作能提昇照護成效。

Medication errors may cause patients severe harm or even death. Health care providers and professionals should take precautions with patient medication safety to promote the effectiveness of medication in illness recovery. This project was designed to promote instruction rates of medication for patients in the surgical ward. The major factors responsible for poor medication instruction rates were the nurses' poor knowledge about medications, a lack of in-service on medication, a lack of patient instruction sheets, a lack of a standardized procedure for inpatient education, and an absence of a monitoring mechanism. The project included educational reinforcement for nurses, development of medication instruction sheets by the pharmacy and the informational technology department, and the establishment of a standardized operational procedure and monitoring mechanism for medication education for surgical inpatients. After implementation of the project, the patient medication instruction rate significantly increased from 72.6% to 94.0%. The score on nurses' awareness of medication improved from 73.3 to 96.7. Patient satisfaction scores on medication instructions, including explanation of medication reactions, adverse effects, and warnings, increased from 84.4% to 91.4%. The success of the project indicates that effectiveness in patient care can be improved through multidisciplinary collaboration.
