The Nurse-Family partnership (NFP) is an innovative evidence-based nursing care model. NFP model aims to improve the lives of new mothers and their infants encountering crisis. NFP model is that specially trained nurses start providing home care to pregnant women before their infants are born. The home care will continue on a regular basis until two and a half years after their infants are born. The NFP model has been implemented in the USA. The studies have shown that the outcomes of the implementation of NFP model include improving pregnant women' health, decreasing the rates of child abuse, neglect and injury, and minimizing post-partum women's risk of re-pregnancy in a short spell. This article also further describes the outcomes of implementation of NFP model in Netherlands, Canada and United Kingdom.
