摘要

本文報告一位27歲男性，柔道後頸部僵硬疼痛、四肢無力，初臆斷急性脊髓壓迫，類固醇治療後，症狀加劇呼吸困難轉入。理學檢查無甲狀腺亢進症狀，實驗室檢查低血鉀，測得甲狀腺激素高而診斷TPP。當TPP發生時，甲狀腺亢進臨床表徵常不明顯或不存在，需早期察覺下列徵象如心搏過速、血壓上升、血清磷降低及尿鉀離子濃度等以鑑別診斷。

Thyrotoxic periodic paralysis (TPP) is a rare complication of hyperthyroidism and is most prevalent in Asian males. It typically presents as proximal muscle weakness, hypokalemia, and signs of hyperthyroidism. We present a case of transient paralysis affecting the upper and lower limbs after trauma that mimicked cervical spine injury clinically in a patient with preciously undiagnosed hyperthyroidism. The case history and a review of the thyrotoxic periodic paralysis literature is presented.
