本文描述運用音樂治療一位重度憂鬱個案,改善憂鬱及焦慮情緒之護理經驗。護理期間為2018年2月12日至5月18日,藉由直接護理、評估、觀察及會談等方式收集資料,經由整體性護理評估後,確立個案有長期性低自尊、焦慮、失眠等健康問題。筆者護理過程中運用音樂治療模式,採取聆聽法、音樂合併運動、歌唱活動介入等,進行個別化音樂治療。藉由音樂治療宣洩壓力及放鬆身心,增進運動持續度,經由音樂討論引導表達內心感受,修正其負向思考,導入腹式呼吸及漸進式肌肉放鬆技巧,成功改善個案憂鬱、焦慮情緒,以及困擾已久的睡眠問題。個案於出院後仍延續音樂治療模式及漸進式肌肉放鬆技巧,穩定情緒及舒眠,得以保持心理舒適狀態,及增進個人的生活品質。
This paper describes the nursing care experience of applying music therapy on a patient with major depressive disorder, thereby improving the patient's bouts of depression and anxiety. The duration of care lasted from February 12 to May 18, 2018, and data were collected by means of direct care, evaluation, observation, and conversation. A holistic patient assessment determined that the patient had chronic low self-esteem, anxiety, and insomnia. The author applied music therapy for personalized treatment with interventions such as listening , music in exercise, and singing. The music therapy was found to relieve the patient's stress and help the patient relax. It also enabled the patient to exercise for a longer period of time. Furthermore, the patient was guided to put their inner feelings into words and correct the negative thinkings through music discussion with the author. These interventions, along with abdominal breathing and progressive muscle relaxation techniques, successfully improved the patient's depression, anxiety, and chronic sleep disorder. After discharge, the patient continued to engage in music therapy and progressive muscle relaxation, which stabilized their mood and improved their sleep. These results have improved the patient's quality of life and helped the patient maintain mental comfort.
