一位周邊動脈阻塞疾病截肢患者之照護經驗

Nursing Experience of a Patient with Peripheral Arterial Occlusion Disease Receiving Knee Amputation

摘要


本篇個案報告在探討一位70歲因周邊動脈阻塞疾病,導致膝上截肢患者之照護經驗。筆者於民國98年9月21日至98年10月1日照護期間,運用Gordon十一項功能性健康型態評估,藉由病史收集、身體檢查評估、會談及觀察方式收集資料,加以整理分析。發現個案有急性疼痛、知識缺失及身體心像紊亂三項護理問題。護理過程中,運用疼痛控制技巧協助個案緩解患肢的疼痛不適;教導個案截肢術後相關照護,增加個案對截肢照護之認知;與個案及家屬共同討論,並擬定個別性的照護,協助個案調適因疾病所導致的身體心像改變,使個案對疾病所造成的截肢有正向的態度,並引導個案配合復健治療,改善疾病所造成的不適,增進日常活動,進而回歸正常的生活及功能。

This article describes a nursing experience caring for a 70-year-old male patient receiving knee amputation for peripheral arterial occlusion disease. During the nursing period which lasted from 2009/9/21 to 2009/10/1, the author assessed the patient's health condition by interview, observations, medical history and physical test assessment and Gordon's 11 functional health patterns assessment guide. The client had three nursing problems: acute pain, knowledge deficit and body image disturbance. During the nursing process, individual intervention was aimed at decreasing pain and increasing his level of physical functioning and comfort, educating her how to care for surgical wound as well as caring for himself in other ways. The individual nursing care plan involved holding discussions with the patient and his family on how to adjust to his new body image, improve attitude towards amputation, and improve his physical functioning and level of comfort by participating in a rehabilitation program. The patient could adapted daily life and performed normal functions.
