Wolf-Parkinson-White syndrome/WPW pattern: flight safety considerations

摘要


沃夫-巴金森-懷特症候群(Wolf-Parkinson-White syndrome,WPW症候群),又名預激症候群(pre-excitation syndrome)是心律不整常見的原因之一,其併發症可能導致心悸甚至昏厥,最嚴重甚至會造成猝死。本文就我國空軍、美國空軍及海軍教範做比較,探討WPW症候群及WPW型態的心電圖在航空醫學上的考量以及缺點免計的規範。在航空醫學的考量上,我們主要關注其復發率以及治療的效果,經研究統計顯示在經過及早發現及適當治療後都能夠有很好的預後。目前治療主流是建議在電生理檢查發現副傳導路徑(accessory pathway)後,針對其進行心導管射頻燒灼術(radiofrequency ablation)治療,術後經觀察無復發可缺點免計,讓飛行員得以重返飛行線發揮所長。

WPW syndrome (Wolf-Parkinson-White syndrome), also known as pre-excitation syndrome, is one of the common causes of arrhythmia. Symptoms can include palpitations, syncope and even sudden cardiac death. WPW pattern refers to the unique EKG finding but without clinical symptoms. This report compared the aeromedical consideration and waiver guidance of WPW syndrome/WPW pattern in our country and United States Air Force, Navy and Army. Electrophysiologic study and catheter ablation of accessory pathways become an established mode of therapy for WPW syndrome/WPW pattern ECG. The aeromedical consideration mainly focused on the recurrence rate after radiofrequency ablation and therapeutic effect of medicine. Research has shown that WPW syndrome/WPW pattern ECG would reach a good outcome after early diagnosis and treatment. A waiver may be considered under close observation without recurrence after curative therapy.
