新型醫療技術之擴展-腹腔鏡相關手術為例

Diffusion of New Medical Technology-laparoscopy Related Surgery

摘要


自1987年全世界第一台腹腔鏡膽囊切除手術誕生後,經過十幾年不斷地器械改良與手術技術進步,腹腔鏡手術及衍生的內視鏡微創手術,已漸漸地取代一些傳統手術。機械手臂輔助下,腹腔鏡膽囊切除手術於1997年首度完成後,外科醫師可以透過電腦螢幕來操作機械手臂來完成手術。2001年,全球首例藉由機械人手臂輔助下,透過電腦畫面操作,橫跨大西洋的遠距腹腔鏡膽囊切除手術誕生了。本文在探討腹腔鏡手術、機械手臂輔助下之腹腔鏡手術及遠距手術,三項外科關連技術,在外科醫療市場的擴散、演進及阻力。腹腔鏡手術,以腹腔鏡膽囊切除手術為例,目前於外科醫療市場上,除了少數落伍者未採用外,其餘大都採用此項技術,來取代傳統的開腹膽囊切除手術,以治療膽結石合併急、慢性膽囊炎病人。利害關係人是可以影響腹腔鏡手術的推廣。相對於腹腔鏡手術之市場成熟擴散,機械手臂輔助下之腹腔鏡手術及遠距手術,現階段僅止於外科醫療界,少數創新者或早期有興趣者之試行採用。成本過高,手術中迅速、無誤的資訊及網路傳輸,國際性、具公信力的醫療管制機制的建立,是機械手臂輔助下之腹腔鏡手術及遠距手術,全球國際醫療市場導入過程中,極待克服的三大難題。

The first laparoscopic cholecystectomy was performed in 1987. The endoscopic surgery has become one of the standard operations in many fields of surgery after decades of years. Robot-assisted surgery had been advocated since the first robot-assisted laparoscopic cholecystectomy was introduced in 1997. In 2001, the first transcontinental robot-assisted remote telesurgery was done by Dr. Marescaux. Eighty-five percent of cholecystemy used in treatment of gall stones with cholecystitis is done laparoscopiclly. However, the robot-assisted laparoscopic surgery and telesurgery have been limitedly applied in surgical fields. The diffusion of new medical technology can be influenced by many factors, e.g. stakeholder. High cost, more rapid, accurate telecommunication and establishment of international regulation were main issues to be faced in the diffusion of these new medical innovation technologies.
