Multiple chronic diseases are common among residents living in long-term care institutions. Unexpected interruption of treatment or drug replacement is very likely to happen when they are transferred to the hospital because of acute symptoms, or discharged back to the agency. These situations are not conducive to disease control, and may easily lead to health deterioration. Pharmacist intervention in the continuum of care can help maintaining medication consistency during care settings conversion, which is an important part of patient safety. This case report illustrates an example of how pharmacist intervention maintains the continuity of medication in a regional hospital in Taipei.
