Evaluation of Breast Supplying Arteries Using Color Doppler Ultrasound in Patients with Breast Cancer

摘要


目的:以超音波評估乳房的血管差異。方法:收集30位正常受試者及30位術前乳癌患者。以超音波量測雙側乳房動脈,含內乳動脈(IMA)、胸尖峰主幹動脈(TAT)、胸外側動脈(LTA),測量其血管直徑及血流因子(含:收縮峰期速度、舒張末期速度、阻力指數、及搏動指數)。比較乳癌病人與正常人採用Mann-Whitney U test。乳癌病人之兩側血管比較則用Wilcoxon signed-rank test。結果:在癌症病患,患側胸LTA 直徑顯著比健側來的大(p=0.01)。而當腫瘤小於兩公分,患側的LTA 直徑仍比健側顯著(p=0.04)。當腫瘤大於兩公分時,IMA的直徑患側顯著大於健側(p=0.02)。在內側腫瘤,IMA的直徑比健側大(p=0.03)。血流因子在統計上無顯著性差異。結論:在血管的直徑上(特別是LTA),乳癌患側與健側有顯著差異,血管增大的方位與腫瘤位置有關。

Objectives: To evaluate the difference of gross vascularity between cancer patients and normal controls using the color Doppler ultrasound. Methods: Thirty patients with breast cancer and thirty normal controls underwent ultrasound examinations were enrolled. Gross vascularity was evaluated by measuring the diameter and flow parameters (peak systolic velocity, end diastolic velocity, resistant index and pulsatility index) of three supplying vessels: lateral thoracic artery (LTA), thoracoacromial trunk (TAT) and internal mammary artery (IMA). The Mann- Whitney U test was used in the comparison between cancer patients and normal controls and the Wilcoxon signed-rank test was used in the comparison of bilateral breasts in breast cancer patients. Results: No statistical differences between the cancer patients and normal controls of all parameters were noted. However, the lesioned breasts were characterized with larger diameter of LTA as compared with the normal breasts (p=0.01). Lager LTA diameters were observed in the lesioned breasts compared with the normal breasts when the tumor sizes were smaller than 2 cm (T1 stage, p=0.04). Larger IMA diameters were also noted in the lesioned breasts compared to normal breasts when the tumor was larger than 2 cm (p=0.02). Significant LTA diameter difference existed in tumors located at outer breasts (p=0.04) and IMA diameter difference in tumors located at inner breasts (p=0.03). The flow parameters showed no differences between groups. Conclusions: The diameters of the supplying vessels (especially the LTA) of the breasts were different bilaterally in cancer patients and the enlarged vessels may be related to tumor locations.
