一位年輕女性活體捐贈肝臟移植手術之手術全期照護經驗

A Perioperative Nursing Experience of a Young Woman Undergoing the Surgery in Living Donor Liver Transplantation

摘要


本文描寫一位30歲女性,接受活體捐贈肝臟移植手術之手術全期照護經驗。護理期間為2016年5月5日至5月18日。術前訪視會談中發現個案因家庭成員因素,有著捨我其誰的使命感,經過家庭決策後卻仍然對捐贈肝臟手術存有疑問,想捐又害怕,想以一己之力善盡孝道,擔心不捨父親受疾病之苦,經歷內心焦慮,擔心手術安全、術後疼痛、傷口疤痕等,天人交戰矛盾的情緒。藉由護理評估確立護理問題:焦慮、潛在危險性手術情境傷害、急性疼痛。護理期間筆者與個案一開始透過多次訪視,傾聽個案需主訴,建立良好信任關係,提供合適的資訊以增進對活體肝臟移植手術的瞭解,鼓勵說出想法及需求給予心理支持及陪伴、設計個別性護理措施,與家屬及醫療團隊陪伴個案一起面對焦慮帶來的心、生理障礙,使其安穩情緒及信任,一起渡過手術過程,以健康角色順利返回社會工作崗位。

This article describes the peroperative nursing experience of a 30-year-old woman who had undergone the surgery in living donor liver transplantation.The nursing period was from May 5th to 18th, 2016.The author discovered the case had a sense of mission because of family members during pre-operative interview. After family decision-making, the doubts about donating liver surgery still existed. The case experienced anxiety and quite contradictory emotion; she wanted to donate her liver to her father but fear, and wanted to fulfill filial piety as well as worried that her father suffered from the disease. Also, worrying about surgical safety, postoperative pain, and the scar. Anxiety, potential risk for operative injury, and acute pain were confirmed by nursing assessment. During the nursing period, a trust relationship was established through frequent visiting and listening to the case's complaint, moreover, providing appropriate information to enhance understanding of living donor liver transplantation, encouraging the expression of thoughts and needs, providing important psychological support and accompany, and designing individual care plans and interventions. With the company of medical team and family members, the case faced the psychological and physical obstacles brought by anxiety and passed the surgery; the case felt relieved and reliable and returned to work with health.
