照顧一位長期血液透析患者心理社會問題之護理經驗

摘要

本篇報告主要在探討一位54歲女性，末期腎病患者持續十六年多接受血液透析所引發身心健康問題之護理經驗。筆者於2003年9月2日至2004年3月30日期間，利用觀察、會談、傾聽，並根據血液透析記錄單及每月定期檢驗報告等方式蒐集資料，採Gordon十一項功能性健康型態爲評估工具，結果發現個案有慢性疼痛、疲憊、無望感等護理問題。在護理過程中運用希望建立之照護技巧及生命回顧，引導個案正向社交活動，並建立良好的支持系統，改善和他人的互動，協助個案適應疾病帶來的身體變化，最後能獲得心靈平靜，重新面對生活。

The purpose of this article is to describe a nursing experience with a female, 54 years old, end stage renal disease with hemodialysis for 16 years about psychosocial problem. The author assessed the patient's health condition by observation, interview, listening, medical history, hemodialysis sheet, laboratory report of every month, and the Gorden functional health assessment guide, from September 2, 2003 to March 30, 2004. The client had three nursing problem: chronic pain, weariness, and hopelessness. The author used nursing process, the skill of hope establish and life review to assist the patient to forward social activity, set up good support system, improvement of relationship with other persons, and help to adapt to the disease. Finally, to set up the elient, peaceful of soul and face life again.
