運用羅氏理論於一位外籍配偶遭受婚姻暴力之急診護理經驗
Applying Roy's Model in Emergency Room Nursing Caring for a Immigrant Who Was a Victim of Domestic Violence
摘要
本文為描述照護一位遭遇婚姻暴力之越南籍配偶到急診室求醫的護理經驗，個案由越南籍朋友陪同就醫，主訴跌倒受傷，一直到診治結束後才向護理人員表示自己被家暴的過程。護理過程2009年4月18日，過程中提供個案一個隱密安全的空間提供生、心理上的支持，避免二度傷害，筆者應用羅氏適應模式搭配客觀觀察、會談及傾聽等方式收集資料，確立個案健康問題為『疼痛』、『恐懼』、『無望感』，在護理過程中除給予醫療處置外，如：通報、驗傷、開立診斷書等，並轉介社工結合社區力量，協助個案連結外界資源與減少婚姻暴力的衝擊，還協助解決後續返回家庭需面對的問題，改變其被施暴後的經驗，終於成功達到去除恐懼、無望感的回饋結果，最後使得個案社會行為技巧提升並增加自我照顧與適應能力。

This article describes the emergency room nursing care of a victim of domestic violence who immigrated to Taiwan by marriage. She first reported her injury as an accidental fall, but later stated that her injury was caused by her husband. The nurse used Roy's model, guided interview, listening and observation conducted a thorough assessment. The patient's major health problems were pain, fear and hopelessness. Nursing interventions included an empathetic attitude to support her emotional needs, response to her questions in an objective manner, and offer of a safe and private environment to maintain her physical and mental health. Social services provided the patient with community resources as a means of continuous support. The goals were to lessen the harm from the abuse experience, and to promote the patient's social and self-care abilities.
