護理人員高齡照護能力培訓課程之成效探討

Effectiveness of Geriatric Training Program in Enhancing Geriatric Care Competencies of Hospital Nurses

摘要


目的:本研究目的為建構護理人員高齡照護能力培訓課程並進行成效評值。方法:採類實驗研究設計,以南部某醫學中心臨床工作兩年以上護理人員為對象,由單位護理長推派參與培訓課程者為介入組,對照組則為同單位年資層級相似之未參與培訓課程者。利用基本資料表(人口學因素、工作因素、教育因素)、護理人員對老人照護知識、態度與臨床高齡照護能力量表於課程前(前測)、課程結束(後測)及6個月後進行資料收集。資料以廣義估計方程式進行分析。結果:總計124位護理人員參與研究,介入與對照組各為62位。介入組自覺高齡照護能力分數於後測顯著高於前測,且課程結束6個月後分數亦高於前測(74.29±9.72分v.s. 61.37±12.77分,p<.01;73.33±8.01分v.s. 61.37±12.77分,p<.01),然6個月後與後測間並無顯著差異(p=.27)。另對照組自覺高齡照護能力分數於課程結束6個月後優於前測及後測(71.28±9.12分v.s. 64.23±11.20分,p<.01;71.28±9.12分v.s. 66.08±8.80分,p<.01),但後測與前測間並無顯著差異(p=.15)。結論:本研究發現高齡照護能力培訓課程可有效增進護理人員之自覺高齡照護能力,且只要針對少數人進行培訓課程即可提升整體高齡照護能力。建議未來醫療院所可將高齡照護能力培訓課程納入政策面推動,以提升護理人員高齡照護能力。

Objectives: The purpose of this study is to develop a geriatric training program for hospital nurses and to evaluate the effectiveness of the training program in enhancing the geriatric care competencies of the trainees. Methods: The quasi-experimental research design was adopted. Nurses with over two years' clinical experience at a medical center in southern Taiwan were recruited and divided into two groups. The nurses in the experimental group received the training program, while those in the control group did not. Data were collected using a structured questionnaire incorporating demographic variables, nursing work-related variables, variables related to geriatric education, geriatric care knowledge scale, attitude toward geriatric care scale, and self-reported geriatric care competency scale. The structured questionnaire was collected before, after and six months after the training program, and data were GEE (generalized estimating equation) analyzed. Results: A total of 124 nurses were recruited (62 for the experimental group and 62 for the control group). The self-reported geriatric care competency in the experimental group appeared to improve after the training program (74.29±9.72 v.s. 61.37±12.77, p<.01) and six months after training program (73.33±8.01 v.s. 61.37±12.77, p<.01). However, the scale of improvement in competency showed no significant difference after the training program and six months after the training program (p=.27). The control group, on the other hand, reported improved geriatric care competency six months after the training program (71.28±9.12 v.s. 64.23±11.20, p<.01), but there was no significant improvement right after the training program (p=15). Conclusion: As indicated by the study results, the geriatric care training program developed by the study is capable of enhancing hospital nurses' self-reported geriatric care competencies. Moreover, only a minority of the nurses need to receive the training to help improve the overall geriatric care competencies of an entire hospital. Hospitals are accordingly recommended to develop and promote geriatric training programs so as to help enhance the quality of their geriatric nursing care.
