Primary sacral epidural lymphoma: A case report and literature review

Non-Hodgkin's lymphoma is a lesion in the spine, and this condition is not common. In particular, lumbosacral area epidural lymphoma is extremely rare. In this study, we presented a case of a 31-year-old man with primary sacral epidural lymphoma who had lower back pain with numbness in the buttocks and right posterior thigh. The symptoms were not relieved with flexion or extension, which is not a typical presentation of spine degenerative disease. Magnetic resonance imaging (MRI) result showed a lesion occupying the spinal canal from L5 to S3 and extending to the presacral area through the neural foramen of L5, S1, and S2. To relieve symptoms, partial laminectomy at L5/S1 was performed, and open biopsy was conducted to obtain tissue samples. The pathological diagnosis was diffuse large B-cell lymphoma. He received chemotherapy with rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone, and MRI result showed remission of the lymphoma. The patient lived well and was symptom free after surgery and 36 months of treatment.
