Ligamentum flavum ganglion cyst with coexisting lumbar stenosis treated by fully-endoscopic cyst excision and stenotic decompression-A case report and literature review

Ligamentum flavum (LF) ganglion cyst has rarely been described in the literature, and degenerative spinal stenosis with coexisting LF cyst is even rarer. Decompression of the spinal canal and excision of the cyst can be achieved by traditional open or tubular approaches. However, a fully-endoscopic approach for decompression and cyst excision may also provide a good outcome, and is minimally-invasive. In this report, we describe a 72-year-old female under the impression of L4/5 LF cyst and spinal stenosis, who underwent fully-endoscopic cystectomy and bilateral decompression surgery successfully. After a year of follow-up, the symptoms were improved and the modified MacNab criteria was Good. For elderly patients with other medical comorbidities, percutaneous stenotic lumbar decompression is the least invasive surgery for cyst excision and spinal canal decompression and has a satisfactory outcome.
