遠距健康照護對於糖尿病個案自我照顧之成效－以金門地區為例

The Effectiveness of Using Telehealth Care in Self-Care for Diabetes Case-A Study in Kinmen Area

摘要

遠距健康照護是一種創新的照護模式，不受限於時間及空間，運用資訊通訊科技的輔助，醫療團隊可以提供個別化的完整照護，病人及其照顧者也能即時回饋健康狀況給醫療團隊，因而獲得較好的生活品質。本研究以金門地區某一鄉衛生所糖尿病個案為研究對象，採類實驗研究設計（Quasi-experimental study），隨機選取40位參加者遠距照護服務個案為實驗組與40位一般門診衛教個案為對照組，共計80位，調查期間為107年7月1日至107年12月30日止。資料分析採用SPSS 20.0中文版統計軟體。結果發現（1）個案基本資料與健康指標，實驗組與對照組在年齡、職業別、舒張期血壓、糖尿病自我照顧量表得分有顯著差異。（2）糖化血色素前後測比較，顯示實驗組糖化血色素後測值下降程度比對照組顯著較好。（3）接受遠距照護者，對於遠距照護滿意度分數為普通偏向滿意端。因此，糖尿病遠距健康照護介入後對於疾病控制確實有成效，未來有必要持續執行並推廣，此結果將提供金門或其他地區未來執行遠距健康照護之參考依據。

Telehealthcare is an innovative healthcare model which is not limited by time and space. Through the assistance of information and communication technology, the medical team can provide individual and comprehensive health care based on feedback from patients and their caregivers, hence improving patients' quality of life. A quasi-experimental study was conducted from Sep-01-2018 to Dec-30-2018, recruiting diabetes patients from a public health center in Kinmen County. A total of 80 participants volunteered for the study. Forty of them were assigned to the telehealthcare group, with another forty participants receiving general health education at the center's outpatient department as the control group. Data were analyzed using SPSS 20.0 Statistics software. Results revealed that (1) Significant differences were found between telehealthcare and control groups in age, profession, diastolic blood pressure, and scores for the Diabetes Self-Care Scale; (2) The level of glycated hemoglobin in the telehealthcare group showed a more significant decrease than that in the control group; (3) The score of satisfaction levels by the telehealthcare group range from ＂neutral＂ to ＂satisfied＂ indicating participants were generally satisfied with the intervention. Findings suggested that the intervention of telehealthcare was effective in managing glycemic levels, therefore, this intervention can be extended and promoted. Results of this study can be used as a reference for future implementation of remote health care in Kinmen or other regions.
