提升燒傷中心疤痕重建手術護理指導成效改善專案
Improvement of effects of nursing instruction for scar revision surgery at burn centers
摘要
目的：在提升燒傷中心疤痕重建手術護理指導成效，病人及家屬因對手術資訊不了解，造成取消手術或延遲，導致醫療資源的浪費，現況調查得知，手術護理指導完整性為79%，深入了解發現主要原因：護理人員對疤痕重建手術認知低；缺乏疤痕重建手術護理指導工具；缺乏疤痕重建手術護理指導手冊；缺乏疤痕重建手術傷口圖卡；缺乏疤痕重建手術護理指導標準化作業。方法：增訂疤痕重建手術標準化作業流程、製作手術護理指導手冊及影片、舉辦疤痕手術護理在職教育及建立查核機制、提供傷口進展圖卡。結果：實施後護理人員對疤痕重建手術護理完整性由79%提升至100%，對手術認知正確性由75%提升至94%；燒傷病人及家屬對手術護理指導認知正確性由75%提升至92%，對手術護理滿意度由78%提升至91%，效果顯著提升。結論：專案利用跨團隊會議後取得共識後，建立疤痕重建手術標準化作業流程，舉辦護理人員在職教育課程、病房常規增訂疤痕重建手術護理指導內容，使護理人員及團隊成員在執行手術護理有一致性及完整性。病人術前的焦慮不安感也明顯有減輕情形。本專案執行成效佳，預計日後將專科經驗及護理指導手冊平行展開於整型外科病房施行。

Purpose: This study aimed to improve the effects of nursing instruction on scar revision surgery at burn centers. As patients and their family members lack knowledge about scar revision surgery, surgical procedures are commonly cancelled or delayed, which leads to wastage of medical resources. A survey of the current situation revealed that the completeness of nursing instruction for scar revision surgery was merely 79%, with the main reasons being the nurses' lack of knowledge on scar revision surgery as well as a lack of relevant tools, instruction manuals, wound image flashcards, and a standard operating procedure for nursing instruction on scar revision surgery. Methods: A standard operating procedure for nursing instruction on scar revision surgery was formulated, an instruction manual and videos for nursing instruction were created, on-the-job training in nursing care for scar revision surgery was conducted, a nursing instruction evaluation system was established, and image flashcards showing wound progress were provided. Results: After implementing the aforementioned measures, the completeness of nursing instruction for scar revision surgery by nursing personnel increased from 79% to 100%, and the accuracy of nurses' knowledge on scar revision surgery increased from 75% to 94%. For patients and their family members, the correctness of knowledge on nursing instruction increased from 75% to 92%, whereas post-surgery satisfaction increased from 78% to 91%. These results indicate a significant improvement in nursing instruction for scar revision surgery. Conclusions: After reaching a consensus on the improvement project during an interdisciplinary team meeting, a standard operating procedure for scar revision surgery was formulated, on-the-job training for nursing personnel was conducted, and nursing instruction for scar revision surgery was incorporated into the ward routine in order to ensure consistency and completeness of nursing care by nursing personnel and medical staff. By implementing the above measures, preoperative anxiety in patients was also significantly reduced. In view of the positive results of this project, further implementation in plastic surgery wards through professional experience and nursing instruction manuals is expected in the future.
