Using a Food Monopoly-style Game to Improve Blood Glucose Control in Patients with Type 2 Diabetes

以食物大富翁遊戲改善第二型糖尿病的血糖控制成效

Background: Taiwan has a rich and diverse food culture. For this reason, it is important to assist patients with type 2 diabetes mellitus to manage their daily diets and choose appropriate foods that provide them with the necessary nutrition requirements in accordance with the disease. Objective: This study aimed to investigate the effects of a food Monopoly-style game on improving blood sugar control behaviors in diabetic patients. Methods: A quasi-experimental design was adopted for this study. Patients with recently diagnosed type 2 diabetes, aged 20-70 years, and a glycated hemoglobin value higher than 7.0% were recruited from the Diabetes Health Education Center of a medical institute in Central Taiwan as study subjects. The experimental group (n=49) participated in a food Monopoly-style game for 90 minutes per session; a second group of subjects was recruited into the control group (n=50) and received conventional diabetes education. The blood sugar results of the subjects in both groups were evaluated before and six months after the intervention. In addition, subjects' blood sugar control behaviors were assessed through self-administered questionnaires. Results: The mean age of the subjects was 52.3 years, and the glycated hemoglobin was 9.2%. The glycated hemoglobin of subjects in the experimental group was significantly lower than that of those in the control group at six months (-2.3 vs. -0.7%); furthermore, the frequency (times/week) of blood glucose self-monitoring (+5.6±3.2) and physical activity (minutes/week) (+42.8±23.0) were greater in the experimental group. Therefore, improvements in diabetes-related behaviors of subjects in the experimental group were greater than those of subjects in the control group at six months. Conclusions: Type 2 diabetes patients who received diabetes education using a food Monopoly-style game had superior improvements in glycemic control behaviors compared with patients who received conventional diabetes education. The results of this study can be used as reference for diabetes care professionals to assist patients with dietary management to control blood sugar.

背景:在台灣,飲食文化豐富且多樣,協助第二型糖尿病患者在日常生活中進行飲食管理,配合疾病選擇適當營養是重要的。目的:本研究探討以食物大富翁遊戲提升糖尿病患者血糖控制行為的成效。方法:採準實驗設計。於中部地區醫學中心糖尿病衛教中心招募最近診斷為第二型糖尿病患者、年齡在20-70歲之間且糖化血色素高於7.0%者為研究對象,實驗組(n=49)以團體方式進行一次食物大富翁遊戲,每次90分鐘;另招募研究對象採常規護理者為對照組(n=50)。二組於收案後與6個月時分別評估患者血糖結果,並以自填問卷收集病患血糖控制行為。結果:研究對象平均年齡為52.3歲,糖化血色素為9.2%。實驗組在6個月之糖化血色素比對照組顯著降低(-2.3比-0.7%);實驗組的血糖自我監測頻率(次/週)(+5.6±3.2)和體力活動(分鐘/週)(+42.8±23.0)更高。在6個月時,實驗組的糖尿病相關行為改善結果優於對照組。結論:與接受傳統糖尿病教育的患者相比,以食物大富翁遊戲接受糖尿病教育的第二型糖尿病患者在血糖控制行為方面有顯著改善。本研究結果可做為糖尿病照護專業人員協助患者以飲食管理控制血糖之參考。
