摘要

台灣已進入高齡社會，如何活躍老化相關議題已被重視，高齡友善城市、長期照顧計畫亦已施行多年，延緩失能、失智、減少醫療支出、減少家屬照顧壓力，讓長者獲得更完善且有人性尊嚴的照顧，同時也可以減輕照顧者負擔，以巷弄長照站為基地，強調「社區」負擔照顧責任，進而達成「在地老化」為終極目標之長照2.0施行計畫，「全民健康保險居家醫療整合照護計畫」之「居家醫療」、「重度居家醫療」及「安寧療護」三照護階段銜接長照2.0，提升照護連續性，兩者可以無縫接軌，讓長者在家安老。

Taiwan is an aged society. People pay more attention to the issues, such as active aging. Some projects have been implemented for several years; for instance, aging- friendly city and long-term care. These programs aim to postpone the age of having incapability and dementia, reduce the caring pressure of the family, and help the elderly to be taken care of more properly with dignity. Meanwhile, in order to decrease the burden of the caregivers, care centers at the neighborhood level emphasize the responsibility of the care at the neighborhood. By doing so, the long-term care plan 2.0 hopefully can achieve the ultimate goal - aging at home. The 3 stages of care, including ＂Home Health Care＂, ＂Serious Home Care＂, and ＂Hospice Care＂ in the ＂National Health Insurance Home Medical Integrated Care Plan＂ are connected to the long-term care plan 2.0 to improve the continuity of care and let the elderly to age at home.
