以家庭系統理論評估與介入憂鬱自殺病人家庭的個案報告

摘要

筆者長期在精神醫療領域進行家族治療工作，因著病人生病、自殺行為、情緒失控等狀況，而有機會接觸病人與家庭。醫療文化較以病人為中心，在協助病人因為疾病／障礙導致功能退化影響時，往往期待家庭給予支持與協助。憂鬱自殺病人出院前尤其會讓家屬感到不安惶恐，家屬需要擔任監測病人自殺風險的重要照顧者。本文將以一個有重度憂鬱症、因自殺進入急性病房的病人家庭為例，但並非呈現整個家族治療的歷程，而是以家庭為整體的觀點，生物－心理－社會模式的視角來理解此病人與家庭，並說明以家庭系統理論的觀點如何在提升病人所處家庭環境的保護因子、降低病人憂鬱自殺風險時，同步關懷家屬的照顧負荷與家庭關係幸福感。

Practicing family therapy in psychiatry field long-termly, the author often contacts patients and families because of their disease, suicidal behavior or unstable emotions. Based on patient-centered culture in hospital, medical professions often guide family to support patients, monitor suicide risk of patients, increase family anxiety before patients' discharge. This paper is a case report, based on family system theory, family as a whole, and bio-psycho-social model of understanding a major depression patient and her family, increasing family protective factors, reducing suicide risk of a major depression patient, and caring family burden and family happiness at the same time.
