因應新冠肺炎建立遠距看診之研究

Research on Needs for the Telehealth Service due to Covid-19

摘要


台灣的健保制度是一個成功的制度,但在低費率與高便利性的背後,也出現過度依賴醫療資源的社會問題。隨著2020年COVID-19的疫情在全球蔓延,遠距醫療(Telemedicine)的重要性又開始浮上檯面,而同時這也是解決台灣民眾對於醫療資源過度依賴的契機。本研究從民眾端的需求出發,由下至上(botton-up)的建構出根基於台灣、與實務契合的遠距醫療發展策略,研究之進行首先爬梳現行國內外遠距醫療之相關理論文獻,根據後續資料收集與分析之需求,彙整有意義之文獻資料初步擬定問卷構面與題目。接續研究進行多場專家訪談,針對問卷進行修編,並透過電話訪談方式向全國各地發送電話訪談邀請。最後進行統計分析與專家座談,並由研究團隊進行綜整總結。研究發現,遠距醫療的發展除了硬體層面的進步外,尚有需多潛在的問題有待解決,涵蓋經濟、操作、法律及安全。資訊安全則是另一個無法避免的挑戰,衍生的法規或評鑑,也增加了醫院推動遠距醫療的壓力。而透過民眾的觀點,研究發現,時間成本不管是來自交通距離或是等候掛號看診,是亟需改善的項目。「操作人員」、「醫療專業」則是遠距醫療流程環節中,民眾最關切的部分。在政策建議方面,研究提出結合部立醫院的轉型,並搭配長照議題進行政策擬定的遠距醫療發展總體策略,而在具體環節上,本研究建議政府應抓住以下三個政策重點,包括:建構指引與認證標準、資訊安全、以及遠距醫療推廣平台。

Research has found that in addition to hardware advancements in the development of telemedicine, there are still many potential problems that need to be resolved, covering economy, operation, law, and safety. Information security is another unavoidable challenge. Derived regulations or evaluations have also increased the pressure on hospitals to promote telemedicine. From the public's point of view, research has found that time cost, whether it comes from transportation distance or waiting for registration, is a project that needs urgent improvement. "Operators" and "medical professionals" are the most concerned part of the people in the remote medical process. In terms of policy recommendations, the study puts forward an overall strategy for telemedicine development that combines the transformation of ministry-owned hospitals with long-term care issues for policy formulation. In terms of specific links, this research suggests that the government should focus on the following three policy priorities, including : Construct guidelines and certification standards, information security, and telemedicine promotion platforms.
