中西醫聯合治療小兒妥瑞症合併過敏性疾病之病例報告
Combination of Traditional Chinese Medicine and Western Medicine for Pediatric Tourette Syndrome With Allergic Diseases: A Case Report
摘要
妥瑞症（Tourette syndrome）是小兒神經科門診中常見的慢性疾患，中西醫合併治療也逐漸成為趨勢，目前已有不少病例報告，分別探討兒童過敏性疾病與小兒妥瑞症。本病例為11歲8個月的男性妥瑞症患者，過去病史患有氣喘及過敏性鼻炎。8歲開始出現清喉嚨聲。儘管自9歲半服用西藥安立復（aripiprazole），三個月後仍復發，出現動作型抽動症狀，到中醫兒科門診求治，經中西藥治療後，聲語型抽動改善，動作型抽動減少，耶魯抽動症嚴重程度量表（Yale Global Tic Severity Scale）分數降低。最後討論以實證醫學回顧妥瑞症及過敏性疾病之關聯，呈現出不同於傳統中醫對於小兒抽動症的想法，本病例藉由結合現代醫學研究及中醫臟腑辨證，提出治療經驗與療效。

Tourette syndrome is a common pediatric neurodevelopmental disorder. Combination treatment with traditional Chinese medicine and Western medicine has gradually become an increasing trend. So far, numerous case reports have discussed pediatric allergic diseases and Tourette syndrome separately. Herein, we report the case of a boy aged 11 years and 8 months with a history of asthma and allergic rhinitis who presented with Tourette syndrome. At the age of 8 years, involuntary throat clearing was noted. Despite the patient taking the Western medicine aripiprazole since the age of 9.5 years, motor tics recurred after 3 months. Therefore, the patient visited the Chinese medicine pediatric clinic for consultation. After the patient started a combination of Chinese and Western medicine, both vocal and motor tics improved. His Yale Global Tic Severity Scale scores also decreased. Finally, we reviewed the association between Tourette syndrome and allergic diseases from the evidence database, which provided different perspectives on the use of traditional Chinese medicine for pediatric tics. This case report presents an effective treatment experience based on a combination of contemporary medical studies and traditional Chinese medicine based on bowel-viscera pattern identification.
