中醫治療小兒抽動障礙合併過敏性疾病之病例報告

Traditional Chinese Medicine for Pediatric Tic Disorder with Allergic Diseases: A Case Report

摘要


抽動障礙(tic disorder)是小兒神經科門診中常見的慢性疾患。中醫的介入給了抽動障礙病童另一種治療的選擇,有些症狀輕微的患者可藉由中醫早期介入將症狀控制。本病例為六歲八個月的女性抽動障礙患者,過去病史包含過敏性鼻炎、胃出血及腸胃炎。自2021年8月4日(6歲4個月)開始出現仰頭扭動的動作,到醫院檢查後並無異常,一個月後因為感冒、眼睛癢及眨眼頻繁而去診所看診,診斷為鼻過敏。同年10月底因腹部收縮動作出現一天而至中醫門診求助,經中醫治療後抽動減少。最後以實證醫學的方式探討抽動障礙及過敏性疾病之間的關聯,本病例藉由傳統中醫臟腑辨證及現代醫學理論的探討,提出正向的治療經驗及療效。

Tic disorder is a common chronic disease in pediatric neurology clinics. The intervention of TCM provides another treatment option for children with tics. Some patients with mild symptoms can control their symptoms through early intervention of TCM. Here we present a case of a 6-year-8-month-old patient with a history of allergic rhinitis, gastric bleeding, and gastroenteritis, suffering from tic disorder. On August 4, 2021 (6-year-4-month-old), the involuntary head-turning movement was noted. There were no abnormal findings on examination. One month later, she went to the clinic for a cold, itchy eyes, and frequent blinking, and was diagnosed with nasal allergy. At the end of October, she went to the Traditional Chinese Medicine Department for help because of abdominal contractions for one day. After TCM treatment, the twitching decreased. We reviewed the association between tic disorder and allergic diseases from the evidence database. This case report presented an effective treatment based on traditional Chinese medicine theory and comprehensive understanding of contemporary medical studies.
