探討護理實習生應用生物科學於臨床照護的態度:以秋葵抗粥狀動脈硬化為例

摘要


背景:近十年來十大死因中,缺血性心臟疾病即位居第二位,動脈血管粥狀硬化的嚴重性是不容勿視。臨床為常見家屬或病患食用秋葵以預防心血管疾病,然而多數臨床護理人員自學校教育以來即缺乏將生物科學應用於臨床照護之經驗,面對病人與家屬的諮詢,無法準確提供對方協助。生物科學乃護理課程中的學習單元,主要是提供學生知識與臨床照護實務結合,若以秋葵為例,臨床上遇到這類詢問時,卻無法提供病人專業的回應,其中原因值得加以研究及探討。 目的:1.探討護理實習生應用生物科學於臨床照護的態度與相關因素。2.探討護理實習生面對病人秋葵萃取相關食品療法的態度與相關因素。3.研究秋葵全果萃物對動脈血管粥狀硬化機轉之影響。 方法:於護理實習生方面,採立意取樣方式,利用問卷調查,探討護理實習生應用生物科學於臨床照護的態度;於生物實驗室內,利用觀察秋葵全果萃取物對oxLDL引起的平滑肌細胞變化,以回應研究問題與假設。 結果:1.護理實習生在應用生物科學於臨床照護的態度得分情形略顯負向,研究對象學習生物科學經驗之壓力指數與應用生物科學於臨床照護的態度具有顯著相關(r=.214,p=.042)。2.護理實習生普遍認同當病人使用秋葵萃取相關食品時,應該評估病人使用的感受,且當病人使用秋葵萃取相關食品時,應該與醫療團隊成員討論。3.在平滑肌細胞實驗中,秋葵全果萃取物抑制oxLDL引起的平滑肌細胞增生的功效則可能來自於秋葵中的多酚成分。 結論:護理實習生學習生物科學經驗之壓力指數偏高,且於應用生物科學於臨床照護的態度顯負向,此值得護理教育者思考,如何設計跨領域課程,將生物科學學習的內容與臨床實務進行結合,於學校教育根本改革著手,使研究對象具備生物科學臨床轉譯能力。實驗結果顯示,秋葵全果萃取物可有效降低oxLD並使增生的平滑肌細胞死亡,此與動脈粥狀硬化機轉過程有關。

Background:In decades, coronary artery disease is second place of 10 leading causes of death. Atherosclerosis is a severe symptom which couldn’t been ignored. In clinical practice, patients and their family used to taking Okra to prevent cardiovascular disease(CVD), but when inquiring related knowledge, lots of clinical nursing are lack of practice of combing biological sciences knowledge and clinical practices. Thus, they couldn’t provide professional and instant help.Biological sciences is part of nursing lessons, teaching nursing students to put knowledge into clinical practice. The reason why clinical nursing couldn’t provide professional advisory, taking Okra, for example, need to be discussed seriously. Aim:1. To discuss attitude of nursing students apply biological sciences to clinical care and correlation factors.2. To discuss attitude of nursing students facing patients’ dietary therapy of Okra extraction and correlation factors.3.To discuss mechanisms of Okra extraction taken in curing Atherosclerosis. Methods:On the nursing students' side, the research adopts Purposive Sampling to discuss attitude of nursing students applying biological sciences to clinical care by questionnaire. On the biological laboratory side, the research observes smooth muscle cells’ reaction induced by Okra extraction of oxLDL. Result:1. Score of attitude of nursing students applying biological sciences to clinical care is on the negative side. The pressure index of objects learning biological sciences and attitude of applying biological sciences to clinical care are significant correlation. (r=.214,p=.042) 2. Nursing students agree that patient’s thoughts about using Okra extraction related food should been evaluated and patients should discuss with the medical team while using Okra extraction related food. 3.In smooth muscle cells experiment, polyphenols of Okra extraction might inhibit smooth muscle cell proliferation induced by oxLDL. Conclusion:Stress index of learning biological sciences is on the high side and nursing students have negative attitude while applying biological sciences in clinical care. This situations is worthy considering when nursing educators design cross-disciplinary lessons, combining biological sciences and clinical practice. In the experiment, Okra extraction could reduce oxLDL and inhibit smooth muscle cell proliferation, and this is related with Atherosclerosis mechanism.
