骨質疏鬆症婦女罹患壓力性尿失禁的風險:全民健康保險資料庫研究

The Risk of Stress Urinary Incontinence in Osteoporotic Female: A Retrospective Cohort Study

摘要


研究目的:本研究旨在確定骨質疏鬆症和壓力性尿失禁對女性的影響。研究假設患有骨質疏鬆症的女性罹患壓力性尿失禁的風險增加。 研究方法及資料:利用回顧性研究使用全民健康保險資料庫2004-2010年的數據。篩選研究族群以鑑定新診斷出患有骨質疏鬆症年齡大於及等於40歲的婦女(ICD-9-CM代碼= 733.0,733.1)。骨質疏鬆症患者共6125人,非骨質疏鬆症患者共12250人。計算新診斷出的壓力性尿失禁發生率,以確定骨質疏鬆症對壓力性尿失禁的影響。我們使用Cox比例風險模型來預測對壓力性尿失禁的相對風險,並使用Kaplan-Meier分析來估計女性壓力性尿失禁的累積發生率。 研究結果:與不同年齡的骨質疏鬆症患者相比,罹患壓力性尿失禁的風險是非骨質疏鬆症患者的1.79倍(95%CI = 1.28–2.51)。與單純性骨質疏鬆症患者相比,病理性骨折患者發生壓力性尿失禁的相對風險更高,但是沒有統計學上顯著的差異。 結論與建議:本研究的數據強調,醫師和護理師應對骨質疏鬆症女性患者進行例行性尿失禁篩檢,並提供適當的轉介、治療以減輕該疾病對患者造成的影響與不適。

Objective:The study aimed to determine the influence of osteoporosis and stress urinary incontinence in women. We hypothesized that women with osteoporosis had an increased risk of urinary incontinence. Methods and Materials:This retrospective study was conducted using data from the Taiwan Longitudinal Health Insurance database from 2004-2010. The study population was screened to identify women (age ≥40 years) newly diagnosed with osteoporosis (ICD-9-CM code = 733.0, 733.1). A total of 6,125 participants formed the osteoporosis cohort, and 12,250 participants without osteoporosis formed the non-osteoporosis cohort. The incidence of newly diagnosed urinary incontinence was calculated to determine the influence of osteoporosis and urinary incontinence. We used the Cox proportional hazards model to predict the effects of urinary incontinence and the Kaplan-Meier analysis to estimate the cumulative incidence of urinary incontinence in women. Results:Participants with osteoporosis had a significantly increased risk of urinary incontinence, regardless of age. We did not observe a higher risk of urinary incontinence in participants with pathological fractures compared to those with simple osteoporosis. Conclusion and Suggestion:Our data emphasized that physicians and nurses should conduct screening for urinary incontinence in women with osteoporosis and implement appropriate interventions to prevent urinary incontinence in women.
