腕關節骨外固定器之創新設計

摘要


進十年來,醫學領域越來越重視與工程的結合,手腕骨外固定器已廣泛應用於腕部的各類骨折,本研究主要是以工程為出發點,導入射出成型為前提,設計一塑膠化的腕關節骨外固定器,在材質上使腕關節骨外固定器無X光遮蔽現象產生,在功能上改進一般上面上腕關節骨外固定器的功能限制,創新設計整合跨腕式與非跨腕式的骨外固定器合而為一體兩用的新型腕關節骨外固定器,有別於一般兩件式的組合。這樣不但擁有功能上的可調性與醫生在臨床上的操作便利性,在X光下亦不會有遮蔽性,充分發揮其復健的功能。本研究同時導入產品開發概念,將CAD、CAE與快速原形等技術應用於本研究中,CAD/CAE可縮短產品開發時間與減少成本,快速原形技術可將設計概念以具體的實體模型呈現出來,達到與醫師溝通的方便以避免設計方向上的錯誤,最後以射出成型之大量生產技術,大幅價低其成本,以減少患者使用上的醫療費用,期望對醫療產業上有所貢獻。 最後經過了整個的創意開發與製作流程後,依照選用的塑膠基材,以實際測量實體原型強度為依據,CAE分析強度為參考,製作出新型的腕關節骨外固定器之實體原型,以達成本研究的最終目的之創新原型開發。

Wrist fixators mainly includes non-spanning configuration and spanning mode. This research is about wrist fixators of spanning mode, which is always applied to common Distal Radius Fracture. Above all, referring to clinical products including TRAUMA-FIX S-20 and ORTHOFIX-PENNIG, then discussing functional design, such as capabilities of telescoping shaft, design intention of ball joints. That can supply to stability and adjustability to prevent the sequelae after operations. But the metal fixators obstructed view of the trauma area from any viewing angle of X-ray and can’t completely benefit from the material’s translucent. Consequently, designing new-type plastic wrist fixators of spanning mode is this research’s purpose. Moreover , dimensions redefined on CAD make the products have similar the mechanical intensity. Then, different conceptual plastic designs are manufactured by rapid prototyping (RP). It is useful to give not only visible aids but physical help and can avoid erroneous designs of kinetic interference. Therefore, this research innovate two design concepts. There are four designs with the other clinical products to analysis. The boundary definition is that less displacement emerge under the range of application of force. Designs which innovate in this research by plastic material of PC and ABS don’t exceed the yield strength under the condition of application of force. The influence results of displacement has such factors as material application of force and geometry of designs, etc. After redefine, the designs not only emphasize the functions, but also use PC+30%G.F. which is plastic material higher in intensity. In conclusion, the displacement can reduce to 1.8 times of the metal material Al of TRAUMA-FIX S-20. This research approach, how is closer the displacement of the metal material effectively, is achieved.
