使用A型肉毒桿菌毒素中胚層療法對於皮膚的效果

摘要


A型肉毒桿菌素近年來在皮膚美容科學中被用來治療臉部皺紋,甚至成為患者接受度最高的微侵入性治療。最近在臺灣陸續有醫師發現在真皮層內注射肉毒桿菌素,也就是中胚層治療(mesotherapy)方式,能讓患者達到皮膚光亮、色素斑改善、毛孔縮小與緊實皮膚的效果。我們嘗試研究真皮層內注射A型肉毒桿菌素,在天竺鼠與自願人體臉部所造成的生物變化。人體實驗部份為雙盲實驗,共8位患者,左右臉同時分別為實驗組與對照組,以真皮層內注射方式施打A型肉毒桿菌素與生理食鹽水後追蹤16週。利用多探頭皮膚生理分析儀(Multi Probe Adapter) 、Visia?系統與傳統照相系統,每4週記錄一次皮膚出油與濕潤度變化、皮膚皺紋、色素等質地的改變。另外在實驗前、後分別取自願者左右臉頰皮膚組織以一般染色與特殊染色來研究皮膚內變化。動物實驗利用5隻雄性天竺鼠,於其背部左右邊分別施打生理食鹽水與A型肉毒桿菌素,每2週取皮膚切片以一般染色與特殊染色來研究皮膚內變化。實驗結果發現臨床上所有受試者與不知情醫師都明顯感受到受試者本身與傳統相片紀錄中,實驗組與對照組的臉部細紋、動態皺紋與皮膚膚質差異。另一方面,組織切片染色下可見注射A型肉毒桿菌素組的膠原蛋白增生與原膠原蛋白增加。多探頭皮膚生理分析儀與Visia?系統的數據也有左右差異,包括出油量減少、皮膚皺紋改善。最後,我們利用黑色素細胞癌細胞株B16F0來研究A型肉毒桿菌素的細胞作用,結果發現A型肉毒桿菌素在生體外會干擾黑色素的生成。本實驗結果顯示,真皮層內注射A型肉毒桿菌素或許可以成為反轉老化現象的有效治療之一。

Type A botulinum toxin has been used in cosmetic dermatology as a nonsurgical and minimal invasive treatment for wrinkles. Recently Type A botulinum toxin is injected into intradermal space as mesotherapy and effects including skin shining、pore size reducing and pigmentation improving were noted by many doctors in Taiwan. We hereby investigate the biologic change after intradermal injection of Type A botulinum toxin to guinea pigs and volunteer people. A double blind evaluation of intradermal injection of Type A botulinum toxin is performed on the left and right faces of 8 volunteers respectively within 16 weeks. We record the skin changes of skin moisture and sebum production、wrinkles 、pigmentation and texture every 4 weeks with traditional photography、Visia? and multi probe adapter(MPA) system. We take skin biopsy from treatment area of volunteer people and study with H&E and special stain. Further, we perform an animal study with 5 male guinea pigs. After intradermal injection with normal saline or Type A botulinum toxin on the bilateral sides of the backs of 5 guinea pigs respectively, we take skin biopsy every 2 weeks in a period of 6 weeks. We study the histopathological change of the skin specimen from guinea pigs and volunteers with Type A botulinum toxin or normal saline. According to the records of traditional photography、Visia? and MPA systems, there is prominent improvement of facial fine wrinkles、dynamic wrinkles and skin texture after intradermal injection of Type A botulinum toxin. On the other hand, the histopathology shows the density of collagen and procollagen increases in the Type A botulinum toxin injection group. Finally, we study the possible cell biologic effect of Type A botulinum toxin in melanoma cell line B16F0 and the preliminary data shows Type A botulinum toxin will interfere the production of melanin in vitro. In conclusion, these results reveal that intradermal injection of Type A botulinum toxin may be one of the alternative methods to reverse the aging phenomena of the skin.
