就國內現行狀況來說,各大小醫療院所內電子化資訊交換機制尚未發展健全,隨著健康資訊通訊標準「健康資訊交換第七層協定(HL7,Health Level Seven)」逐漸為大眾所認可,解決了現行醫院與醫院間電子資料交換上的困難。 本研究之主旨乃是為了解決現行醫院與醫院間轉診上電子病歷交換的困難,並結合健康資訊交換第七層協定,以天主教耕莘醫院為環境,建立一套轉診系統。目的包含:(1)整合院內轉診相關單位作業流程。(2)發展自動化流程。(3)導入適用於台灣本土之HL7/XML轉診格式。另外也利用HL7版本2.4所提供的XML對應方法,將資料編碼成為HL7/XML訊息。 在開發技術上,採用Web Base之使用者介面,並考量到醫院內,不論是診間、門診單位、急診單位或是轉診中心,都是使用獨立的工作平台(個人電腦),因此開發Web Base的使用者介面,可以免除使用者自行安裝轉診應用系統的麻煩,只需要利用瀏覽器,就能夠操作使用轉診的功能。另外,HL7代理人程式(Agent)的實作也為此系統提供了一個高度擴展的平台,系統不但能自動處理訊息的解、編、傳、收動作,對於未來新增其他種類HL7/XML格式訊息亦留下了高度的彈性。
The electronic information exchanging processes have not been developed whole in most of the hospitals in Taiwan yet. But there is a communicating standard “Health Level Seven (HL7)” evolves to be a health information communication standard and becomes popular in the field of health information communication. it gives a solution to the problem of data exchange between two hospitals. The purpose of this research is solving the difficulties of exchanging patient referral information electronically. We adopted “Health Level Seven” in the environment of Cardinal Tien Hospital to develop a patient referral information system. The goals include: 1. Integrating with optional processes about the relative referral departments. 2. Developing automatic referral process. 3. Importing HL7/XML standards suited with Taiwan. We take advantage of the mapping method provided by HL7 version 2.4 and encode the data into HL7/XML message. Because of the users may use patient referral information systems in several distributed workstations or personal computers, we design web based user interface. Web based user interface can solve the problems that users need to install the application software in their own client computers. The users can easily use browser to operate the patient referral function in web based client interface. In addition, the implementation of HL7 Multi-Agent provides an expandable character for system.
