師徒制與知識分享行為對子宮頸抹片判讀品質之影響

The Influence of Mentoring and Knowledge Sharing Behavior on the Pap Smear Interpretation Quality

摘要


目的:探討病理診斷單位實施師徒制的現況,以及在不同師徒類型與師徒功能下進行知識分享行為對子宮頸抹片判讀品質的影響,以提供病理診斷單位作為改善子宮頸抹片判讀品質之參考。 方法:本研究以全國病理診斷單位之細胞醫檢師為研究對象,進行橫斷式立意抽樣問卷調查法。問卷內容包含基本資料、師徒類型、師徒功能量表、知識分享行為量表和子宮頸抹片判讀品質等。共發放問卷405份,回收273份,有效問卷共266份,有效回收率67%。採用SPSS18.0版統計軟體進行資料分析,除了敘述性統計、獨立樣本t檢定、單因子變異數分析、卡方檢定外,並利用羅吉斯迴歸分析檢測師徒制、師徒功能與知識分享行為對子宮頸抹片判讀品質的相關性。 結果:非正式師徒制的子宮頸抹片判讀結果,品質較好的比例高於正式師徒制(p<0.05)。正式師徒制的專業功能高於非正式師徒制(p<0.05)。外部師徒制的角色楷模功能高於內部師徒制(p<0.001)。跨性別師徒制的專業功能高於兩性皆有師徒制(p<0.05)。知識分享行為對子宮頸抹片判讀品質無顯著相關性。透過羅吉斯迴歸分析顯示,非正式師徒制和專業功能對子宮頸抹片判讀品質有顯著預測力,非正式師徒制抹片判讀品質較好的勝算比是正式師徒制的2.78倍(O.R.=2.78,95% C.I.=1.05-7.32,p=0.039);專業功能對子宮頸抹片判讀品質呈現顯著正相關(p=0.005),專業功能分數每增加1分,子宮頸抹片判讀品質較好的勝算比提高1.54倍(O.R.=1.54,95% C.I.=1.14-2.08,p=0.005)。 結論與建議:研究結果顯示非正式師徒制比正式師徒制的子宮頸抹片判讀品質結果較好。因此,建議病理診斷單位鼓勵細胞醫檢師尋找適合個人諮詢的病理醫師或資深細胞醫檢師,以提升個人閱片品質,進而提升單位診斷品質。

Objective:To discuss the status of mentoring in cytopathologic laboratories, and examine the influence of mentoring type and functions along with knowledge sharing behavior on Pap smear interpretation quality, in an attempt to serve as a reference for improving the interpretation quality in Pap smear. Methods:We recruited cytotechnologists of cytopathologic laboratories in Taiwan as subjects. A cross-sectional survey using purposive sampling was conducted in this study. Demographic information, type of mentor, mentoring function scale, knowledge sharing behavior scale and Pap smear interpretation quality were included in the questionnaire. A total of 405 questionnaires were distributed, and 273 responses were received with 266 valid questionnaires (67%). The analysis was conducted by SPSS18.0 for descriptive statistics, independent-sample t test, ANOVA and Chi-square test. Moreover, we used logistic regression to analyze the relationship among mentoring type, mentoring functions, knowledge sharing behavior and Pap smear interpretation quality. Results:The quality of interpretation for Pap smear under informal mentoring was better than formal one (p < 0.05). The vocational function of formal mentoring was superior to informal one (p < 0.05). The role modeling function of external mentoring was better than internal mentoring (p < 0.001). The vocational function of transgender mentoring was superior to bigender mentoring (p < 0.05). There was no significant relationship between knowledge sharing behavior and Pap smear interpretation quality. According to the logistic regression analysis, informal mentoring and vocational function had a statistically significant and predictive effect on Pap smear interpretation quality. The better Pap smear interpretation quality for informal mentoring was 2.78 higher than formal one (O.R.=2.78, 95% C.I.=1.05-7.32, p=0.039). The vocational function had a significant positive relationship with Pap smear interpretation quality (p=0.005). By one point higher in the vocational function, the odds ratio of Pap smear interpretation quality increased by 1.54 times (O.R.=1.54, 95% C.I.=1.14-2.08, p=0.005). Conclusion and Suggestion:According to the research, the Pap smear interpretation quality of informal mentoring is better than formal one. Therefore, cytotechnologists are encouraged to find cytopathologists or senior cytotechnologists for personal consultation to enhance the personal interpretation quality and then improve diagnosis quality of laboratories.
