新式手腕護具對於握力與肌力影響之研究

摘要


腕隧道症候群(Carpal Tunnel Syndrome, CTS)為現今醫學臨床上常見疾病之一,其造成的原因,主要為患者因工作職業與日常活動中過度使用手腕及腕部,因此壓迫手腕之正中神經,使手指及腕部產生酸麻、刺痛等神經壓迫的症狀,嚴重者甚至會影響工作及日常生活等手部活動。現今的電腦技術發展快速,電腦滑鼠已普遍應用於個人生活或職場中,長期以錯誤方式使用滑鼠,易使手腕頻繁彎曲,亦會導致手腕神經受到壓迫,引起上肢肌肉骨骼症狀的機率越大,常見的上肢肌肉骨骼症狀即為腕隧道症候群。如今腕隧道症候群已成為文明病,當手腕出現疼痛或麻痺等腕隧道症候群臨床症狀,臨床上普遍以穿戴硬式手腕護具,亦稱為副木,將手腕固定在自然姿勢,藉此達到緩解症狀並改善握力與肌力之目的,但副木為了提供手腕護具之穩定性,以大面積塑料包覆患處,易使患者感到不適。 本研究以TRIZ理論為工具設計一新式手腕護具,探討新式手腕護具對腕隧道症候群患者握力與肌力之影響。透過整合相關文獻與TRIZ理論,作為本研究設計之依據,設計實體化後,利用肌電儀與握力計量測在操作滑鼠實驗後徒手與穿戴市售型及新式手腕護具之握力值與肌電訊號,並將實驗數據進行魏克生符號檢定分析(Wilcoxon Signed Ranks Test),探討穿戴新式手腕護具在握力值與肌力值有無統計上的差異。最後搭配滿意度問卷調查,了解受測者實際穿戴新式手腕護具之實際感受,作為研究成效之依據。研究結果顯示透過TRIZ理論設計之新式手腕護具於握力及肌力皆有達顯著之差異。

Carpal tunnel syndrome is one of the common diseases in clinical medicine nowadays. The main cause is that the patients overuse the hand and wrist due to work, occupation and daily activities. Therefore, the median nerve of the wrist is oppressed, causing numbness, tingling, and other nerve compression symptoms on the fingers and wrists. Severe cases may even affect hand activities such as work and daily life. Nowadays, computer technology is developing rapidly, computer mouse has been widely used in personal life or workplace. Using the mouse in wrong way for a long time, it is easy to make wrist bend frequently, which will also lead to wrist nerve compression. The higher probability of causing upper limb musculoskeletal symptoms, the common upper extremity musculoskeletal symptoms are carpal tunnel syndrome. Nowadays, carpal tunnel syndrome has become a disease of civilization. When the wrist has clinical symptoms such as pain or numbness of the carpal tunnel syndrome, it is common clinically to wear a hard wrist protector, also known as splint. Fixing the wrist in a natural posture can relieve symptoms and improve grip and muscular strength. However, in order to provide the stability of the wrist protector, the accessory splint uses a large area of plastic to cover the affected area, which is easy to make the patients feel uncomfortable. In this study, a new type of wrist protector was designed based on TRIZ theory to explore the effect of the new type of wrist protector on the grip and muscular strength of patients with carpal tunnel syndrome. By integrating the relevant literature and TRIZ theory, as the basis of the design specifications, after the design was materialized, the grip strength of bare hand, wearable and new wrist protectors was measured by electromyography and grip measurement after the mouse operation. The experimental data were analyzed by Wilcoxon Signed Ranks Test to explore whether there is statistical difference between the grip and muscular strength of the new wrist protectors. Finally, the satisfaction questionnaire was used to understand the actual feeling of the subjects wearing the new-style wrist protectors as the basis of the research results. The research results show that the new wrist protectors designed through TRIZ theory have significant differences in grip and muscular strength.
