運用醫療管理系統改善醫務管理之研究-以大陸某醫療集團為例

摘要

隨著科技發展的腳步，客戶為主的服務概念與全球化經濟的思維快速的影響到每一個不同的產業。醫療產業不外如是，為了滿足經營管理之所需，醫療產業自1960年起引進了企業資源規劃系統（Enterprise Resource Planning，ERP）概念加以整合醫務管理，藉以強化醫療院所的競爭能力。但也使醫療產業的特殊性故在醫療管理系統（Healthcare Information System，HIS）在規劃、設計、研發與運用上，無論在開發時程、使用便利性與投資報酬率中屢屢遭受到管理階層與使用者的抱怨。反觀近年來醫療管理系統的相關研究與文獻，常見各專家及學者針對醫療管理系統功能性，如: 手術系統、護理資訊系統、影像處理系統等等，皆有相關性的研究。但唯獨在醫療管理系統的導入之評估、完善功能性之優先順序、功能性改善之方式較為缺乏，本研究使用科技接受模型(Technology Acceptance Model，TAM)為系統導入前的規畫性研究，並使用聯邦企業架構(FEA，Federal Enterprise Architecture，FEA)與層級分析法(The analytic hierarchy process，AHP)進行醫療管理系統服務組合指標之建構，參考FRM模型(Financial Risk Manager，FRM)修正成為個案醫院專屬之「醫囑分析模型」改善了最迫切需求的「醫囑後續流程管理系統」，並藉由訪談與滿意度問卷確認運用完善的導入醫療管理系統計畫，將有助於改善醫務管理之複雜度。

With the development of science and technology, the concept of customer oriented service and the thinking of globalize economy had affected various industries rapidly; the medical industry is no exception. In order to meet the needs of operation and management, the medical industry has introduced the concept of Enterprise Resource Planning (ERP) since 1960 to integrate the needs for healthcare management and to help strengthen the competitiveness of medical institutions. Due to the uniqueness of the medical industry, the planning, design, development and use in Healthcare Information System (Healthcare Information System, HIS) often suffer the complaints both from the management departments and the users, it happens in the time for development, the ease of use and the return of investment. On the contrary, when probe into the related research and literatures on Healthcare Information System (HIS) in recent years, we can find related researches on the functional aspects in Healthcare Information System (HIS) stated by many experts and scholars, like Surgical System, Nursing Information System and Image Processing System, etc. But in the lack of the evaluation, priorities of functional improvement and improvement in function in the introduction of Healthcare Information System (HIS), this study will use the technology acceptance model (Technology Acceptance Model, TAM) as the planned research prior to the implementation of the system. Then set up the composition index of the medical management system service by using the Federal Enterprise Architecture (FEA, Federal Enterprise Architecture) and analytic hierarchy process (AHP). We modified the "Orders analysis model” by considering the case hospital with the reference of FRM model, to improve the most urgent needs of "Orders follow-up process management system". In the mean time, through interview and questionnaire to confirm that the project of the introduction of Healthcare Information System (HIS) will help to improve the complexity of medical management system.
