膀胱過動症是常見但非致命性的泌尿道疾病,所以易被忽略就醫;雖困擾婦女身體、心理與社交活動,但臨床上婦女較少因此疾病的症狀求診。隨著婦女的生殖泌尿系統之健康議題廣泛討論,此疾病逐漸受到重視。本研究旨在探討罹患膀胱過動症婦女的症狀影響及因應歷程之生活經驗。 本研究採質性研究,以深度訪談法進行資料收集。自民國96年8月至97年1月間,於中部某醫學中心婦產科門診針對膀胱過動症以立意取樣進行收案,藉由研究者與個案一對一的深入訪談,研究者將訪談資料謄寫為逐字稿後,再依據Colaizzi(1978)的資料分析步驟,加以歸類整理。直到資料分析達飽和共收案10人。 研究結果顯示罹患膀胱過動症婦女之共通生活經驗:一、陷入窘境心糾纏,二、生活變調相牽制,三、自我照顧激潛能,四、和平共存希望有。由以上的生活經驗,反應了以「病人為中心」的人性化護理的重要性,協助尋找適當治療與足夠支持的必然性,並提升病患自我照顧與調適的可行性,進而增進病患對自己人生的掌控。 總之,依據本研究結果可以瞭解膀胱過動症婦女的獨特生活經驗,提供醫謢人員以個案的觀點,體會其身心及社交等需求,並針對其生活經驗與需求,給予個別化及人性化的排尿照顧模式,達到整體性的照護目標。
Overactive bladder is a common urinary tract disease but not deathly, so it is easily neglected for medical management. It impacts the physical, psychological and social aspects of women, but they seldom seek for medical treatment. With widespread medical discussions on female genitor-urinary system, overactive bladder is constantly approached and researched. This study aims to investigate the lived experience influenced by the symptoms and course of management of women with overactive bladder. This study is a qualitative research, and data were collected based on purposive sampling in a gynecology and obstetrics department of a medical center in central Taiwan from August 2007 to January 2008. Subjects were one-to-one interviewed. Tape-recorded materials were transcribed and analyzed based on Colaizzi’s methodology (1978). The totals of 10 subjects were included. The results show that the subjects’ lived experiences are categorized into four meaning themes: (1) suffering in this plight, (2) limiting life styles, (3) developing potentials for self-care, and (4) living peacefully with overactive bladder. Their lived experience indicates the importance of “patient-centered” nursing care and the necessity for appropriate treatment and support. They also help patients enhance their abilities for self-care and adaptation, and have a better control of their lives. In conclusion, this study provides essential information to medical professionals involved in urination disorder care by understanding the lived experiences of women with overactive bladder. It is expected that their physical, psychological and social needs are emphasized while making individual micturition caring plan and related researches to achieve an overall caring goal.
