探討數位乳房攝影重照關係影響因素

摘要


根據台灣衛生署100年所發表之公共衛生年報統計,民國99年度癌症死亡人數共計四萬一千零四十六名,癌症即位居國人十大死因第一位,其中女性十大癌症發生率最高者即乳癌(衛生署統計年報,2011)。 乳癌篩檢方式除了簡單的自我乳房檢查及醫師觸診之外,就診斷式的乳癌篩檢方式而言,除乳房超音波外,目前主要以乳房攝影檢測為主要診斷工具。乳房攝影檢查雖說成效最好,但會使受檢者於檢查時吸收輻射劑量,國內外許多文獻指出,過多輻射劑量的吸收會提高罹癌的風險,尤其是因乳房主要組成的結構為軟組織,若不慎吸收過高的幅射劑量,較一般身體組織更容易產生病變。因此,倘若因各種失誤導致影像須重新拍攝,不僅浪費醫療資源,對患者再次接受輻射劑量也是一種相當性危害。 本研究為探討乳房攝影重照影響因素,藉重照類型分析提出相關建議及改善措施,使受檢者能有更安全的檢測環境。本研究收集中部某醫院100年4月至12月間乳房攝影的全部資料共計3006筆,利用統計分析軟體SPSS18做各項相關分析;分析內容有重照與受檢者年齡、放射工作人員特質、攝影特性等相關性研究。 由乳房攝影影像重照率之研究結果得知,全部乳房攝影影像重照率為3.89%,受檢者年齡與影響影像重照有正相關,重照原因主要為擺位不當所致;在操作人員部份,經邏輯斯迴歸驗證放射人員的工作年資與經驗,亦會影響乳房攝影影像重拍的發生機率。

According to Public Health Annual Report Statistics published in 100 by the Taiwan Department of Health, a total of forty-one thousand and forty-six died of cancer in 2010 and one out of top ten cancers that caused female dead was breast cancer. Cancer has been ranked the first of the ten leading causes of death (Department of Health Statistics Annual Report, 2011). Simple breast cancer screening methods include self-breast examination and physician palpation. In diagnosis of breast cancer screening, mammography detection is the main diagnostic tool, in addition to breast ultrasound. Mammograms, although the most effective, will make the subjects absorb in radiation dose. Many domestic and foreign literatures indicated that the absorption of excessive radiation dose will enhance the risk of cancer. The structure of the breast is mainly composed of soft tissue and is more easily and badly damaged, if accidentally takes too much radiation. If the image must be remade due to the various mistakes, not only will a waste of medical resources be caused, but patients have to undergo radiation dose, which is a considerable hazard. This study is to explore the impact caused by redoing mammography, and through re-photography type analysis to provide suggestions and improvement measures so that patients would have a more secure testing environment. All the mammography information, 3006 pieces, was collected in a hospital in Central Taiwan from April to December, 2011, and analyzed using statistical analysis software SPSS18. Analysis contents include the age of the patients receiving more than one mammography, characteristics of radiation workers, and photography features. The study found that the redoing rate all mammography images is 3.89%. There is positive correlation between the age of the patients and re-photographing. The main reason for re-photographing is the improper position. As to the machine operators, the experience of radiology staff is related to redoing mammography.
