條口透承山穴治療五十肩

Treatment of Frozen Shoulder by Joining ST-38 to UB-57

摘要


五十肩症狀為肩部疼痛、僵硬,外展、內外轉活動受限,在進行日常動作時困擾著患者,中醫辨證五十肩為過度勞累,以致風寒邪氣趁虛侵襲肩部經脈,經脈凝滯而形成肩痛,因為人過中年陽氣虛弱,正氣漸損,故好發於50歲以上。西醫有藥物治療,物理治療包括電療、熱敷及自我運動,手術方面則是直接處理粕連的發炎組織;中醫方面則有服藥、運動治療及針灸治療,1979年世界衛生組織(WHO)發佈針灸治療對五十肩的療效,針灸治療五十肩一般臨床取穴有肩井、肩體及肩體的近部取穴,而在遠端取穴中又以條口(ST-38)與透承山(UB-57)兩穴最為著稱,條口透承山治療時,利用紅外線立體攝影機紀錄受試者手臂外展(abduction)角度的變化,發現受試者不會因為心理療效產生作用。結果針刺條口透承山穴改善外展角度的最大來源,來自肩胛胸廓關節的增加,而非病灶點盂肱關節上,若能氨基酸傷科治療,去除肩關節的沾黏現象,或許帶來更突破性的治療效果。

Symptoms of frozen shoulder, also known as wu shi jian or fifty year old’s shoulder include pain, stiffness and limited ability to abduct, pronate or supinate the arm. Consequently is usually quite distressing for those afflicted to go about there daily activities. Chinese medicine identifies frozen shoulder as a pattern of excessive taxation fatigue leading to wind cold evil qi exploiting vacuity to invade the channels of the shoulder. The wind cold qi blocks the channels and thereby causes pain. It is common for this disease to manifest after 50 years of age when the yang qi begins to weaken and the right qi gradually decreases. Western medicine treatments include drug therapy, physical therapy such as electrotherapy and self exercise, and surgery which involves removing adhesions and inflamed tissue. Chinese medicine treatments include herbal therapy, exercise and acupuncture. In 1979 the World Health Organization released information about the effectiveness of acupuncture in the treatment of frozen shoulder. When using acupuncture to treat frozen shoulder the local points commonly used are jian jing (GB-21), jian liao (SJ-14) and jian yu (LI-15), while among distal points selected tiao kou (ST-3 8) joined to cheng shan (UB-57) is the most famous treatment. To test the effects of joining ST-38 to UB-57 I made use of an infrared stereo camera to record changes in the angle of abduction in the test subject’s arm. The results showed that joining ST-3 8 to UB-57 primarily helped improve the range of motion in the scapulothoracic joints not the joints of the upper arm. If this type of acupuncture treatment was combined with trauma therapy to help break adhesions perhaps even better results could be achieved.
