台灣精準輸血醫學之發展

摘要

精準醫學一直是各方努力的目標，但對輸血醫學而言，二十世紀初的ABO血型配對早已是精準醫學的最佳實例。過去，使用捐血人與受血人交叉試驗相容的血液，是確保輸血安全的首要標準；雖然如此，仍無法避免受血人輸血後被免疫產生異體抗體；今日，為達到預防性及策略性的精準輸血醫學，台灣血液基金會（我們）提供醫院血庫輸血諮詢檢驗服務，並主動建置捐血人血型資料庫，以供應病人抗原陰性／相容的紅血球或血小板血品，有效預防輸血後產生異體抗體；更提供多樣化的成分血液選擇，以降低輸血相關急性肺損傷、非溶血性發熱性輸血反應等各種輸血不良反應的發生率。未來，依據「台灣血液安全監測系統」之病患用血資料分析結果，將可制定更佳的臨床輸血策略，同時輔以人因工程資訊科技，持續朝向零風險、合理的精準輸血努力！

Precision medicine has always been the goal of all parties' efforts. However, for transfusion medicine, ABO blood typing has been the best example of precision medicine since early 20^(th) century. In the past, using cross-matched blood was primary for transfusion safety. Nevertheless it was still possible for recipient to be allo-immunized. Taiwan Blood Services Foundation takes the initiative to build blood donor database that can provide patients with antigen-negative / compatible red blood cells or platelet products. This management effectively prevents allo-immunization. We also provide a variety of blood components in order to reduce occurrence of various transfusion adverse reactions. In the future, based on the data analysis of Taiwan Hemovigilance System and human engineering information technology, we will consistently make efforts to zero-risk, reasonable precision transfusion medicine!!
