以急診工作負荷指數即時監測系統進行急診壅塞管理決策

The Decision Management of Emergency Department Overcrowding by the Real-time Monitoring System based on the Emergency Department Work Index (EDWIN)

摘要


急診是醫療安全網重要的一環,本研究建置採用急診工作負荷指數(Emergency Department Work Index,EDWIN)評估急診壅塞情況的監測管理系統,結合資料庫系統與網頁動態圖表,呈現出具即時性與直觀性的急診壅塞情形,提供醫院各層級的決策者及現場醫護人員可以根據監測管理系統提供的資訊採取適當行動,以提高急診醫療品質進而降低急診壅塞問題。本研究透過急診品質監測管理系統分析急診壅塞的原因,包含不同科別的急診就診人數影響分析、連續假期的急診就醫人數與急診壅塞情形分析、急診醫師人力排班對於急診壅塞影響分析。透過本研究所開發之急診壅塞監測管理系統可更精準提供急診工作負荷即時資訊,並協助醫療照護團隊進行醫療管理決策,進而達到提升急診醫療照護品質之目的。

Emergency department is an important part of the health care safety net. In this study, a monitoring system based on the Emergency Department Work Index (EDWIN) was developed to assess emergency department congestion. In the proposed system, a combination of a high-dimensional database system and web-based dynamic charts presents a real-time and intuitive picture of the emergency department congestion. The hospital decision makers at all levels and on-site health care workers are with the ability to take appropriate actions based on the information provided by the monitoring system. This study analyzes the causes of emergency congestion through the emergency quality monitoring management system, including analysis of the impact of the number of emergency patients in different departments, analysis of the number of emergency patients and emergency congestion during consecutive holidays, and analysis of the impact of emergency physician manpower scheduling on emergency congestion. Through the emergency congestion monitoring system developed by the study, more accurate real-time information could be provided on emergency workload and assist medical care teams in making appropriate decisions to improve the quality of emergency medical care.
