A型及B型肝炎疫苗與慢性腎臟病

摘要


慢性腎臟病患者及透析病患,因有較高B型肝炎的感染風險及形成慢性帶原者的機率,目前各診療指引包括美國疫苗接種諮詢委員會建議(Recommendations of the Advisory Committee on Immunization Practices)與改善全球腎臟病預後組織指引(Kidney Disease: Improving Global Outcomes Guideline)皆建議做B型肝炎疫苗之預防接種以降低發病率及死亡率;而針對A型肝炎,目前僅建議針對高風險族群進行接種,包括:患有慢性肝病、曾經移植肝臟的病人、男男間性行為者或靜脈藥癮者及赴流行地區者。應考慮慢性腎臟病患者個別狀況,給予適當的肝炎疫苗施打建議。

Patients with chronic kidney disease (CKD) and end-stage renal disease have a higher risk of hepatitis B infection and progress to the chronic carrier. According to the Advisory Committee on Immunization Practices and Kidney Disease: Improving Global Outcomes guidelines, hepatitis B vaccination was advised for those patients to decrease associated morbidity and mortality. Hepatitis A vaccination was only recommended for high-risk groups, including patients with chronic liver disease, liver transplant recipients, men who have sex with men, intravenous drug users, and epidemic-area visitors. For CKD patients, the recommendation of hepatitis vaccinations should be individualized.
