肌肉電刺激對慢性阻塞性肺病患者下肢運動的成效

Effects of Lower Limbs in Chronic Obstructive Pulmonary Disease Patients with Electrical Muscular Stimulation

摘要


慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease; COPD)主要臨床表現以呼吸道功能下降為主,但是當COPD個案之嚴重度分級處於嚴重~極重度時,常會伴隨有骨骼肌肌肉失能的問題,將會大幅影響患者的運動能力。從19世紀至今,肌肉電刺激漸漸的發現到對於被刺激的肌肉有明顯的進步與改善,已經被切除神經傳導或是健康人的肌肉電刺激會增加肌肉質量、運動員使用後會改變肌肉張力等等。此篇綜合論述文章主要是整理相關於COPD患者使用肌肉電刺激(Electrical muscular stimulation; EMS)後肌肉強度(strength)、運動能力(Exercise capacity)、呼吸困難程度與肺功能的改變。不論使用時間、與肌肉電刺激設定,對於不同種族與疾病嚴重度分期的COPD個案,肌肉電刺激對於隨機且樣本數較大的研究的確是有著明顯的效果。COPD患者在使用後的確有明顯的改善患者肌肉力量、運動能力與生活品質。

Chronic Obstructive Pulmonary Disease (COPD) patients are suffered from airway flow limitation. Muscle mass and skeletal muscle dysfunctions are considering that influencing exercise tolerance and degree of fatigue in patients with advance COPD. Electrical muscle stimulation (EMS) began in 19th century. Until now, electrical muscle stimulation (EMS) would be considered to be an new strategy of pulmonary rehabilitation for COPD patients. In this article, we review the literatures that published prior to September 2011 were addresses the effects of EMS, muscle strength, exercise capacity and pulmonary function. They were identified by searching Pubmed, Highwire and Science Direct. All literatures reported significant improvements in muscle strength, exercise capacity and pulmonary function. Anticipated limitation was number of study. Although, the disparity in duration of training, setting of EMS, races and severity, but EMS seems well tolerated and effects in this articles.We concluded that training improved the exercise tolerance, muscle strength and quality of life in patients with COPD. These data suggest that EMS training might enhance exercise tolerance and improve pulmonary function in patients with advanced COPD.
