台灣“出院準備服務個案管理師”之工作現況

Job Contents and Work Conditions of the Discharge Planner in Taiwan

摘要


本研究的目的在於暸解台灣出院準備服務個案管理師(以下簡稱個管師)之工作內容、執行狀況與其相關影響因素,再依實証基礎進一步評估其專業需求。本研究為橫斷式調查法,對象為台灣地區88家由護理人員擔任出院準備服務業務之個管師;問卷共發出153份,回收率88.2%(135份)。研究結果發現:一、台灣地區個管師依個案管理模式推行出院準備服務之工作內容包括六項:「個案篩選」、「個案照護計畫與執行」、「照護品質之監控與評價」、「跨專業服務與協調」、「照護資源之利用與管理」、與「轉介機構之照護品質」;其中「轉介機構之照護品質」與「照護資源利用與管理」二者為個管師執行狀況頻率較少的部分。二、整體工作內容執行狀況之預測因子為「機構能整合資源服務網絡資訊」、「甄選個管師考量人格性向特質」、「從未接受過出院準備服務在職訓練」及「機構能成立跨專業團隊」,可解釋變異量為28%。本研究建議國內應建立完整的個管師培訓教育訓練,可依研究結果歸納出之六項工作內容,並制定較一致之工作模式,以提供個管師依循,使能發揮工作效能並保障個案權益。

The purpose of this study was to understand the job content and work conditions of the discharge planner in Taiwan, and factors that may be associated with their job performances. A structured-questionnaire was posted to 153 eligible subjects and the response rate was 88.2% (n=135). There are six groups of job contents in discharge planning identified using factor analysis. The six groups are “case screening”, “care plan and plan implementation”, “quality of care monitoring and evaluation”, “multidisciplinary service and coordination”, “resources utilization and management”, and “quality of care of the referred agency”. Of those job contents, “quality of care of the referred agency” and “resources utilization and management” are being performed less frequently. In addition, “integrated resources & information network”, “discharge planner selection with personality consideration”, “never received education in discharge planning”, and “establishing a multidisciplinary team” are important predicting factors for better job performance. In conclusion, the six groups of job content identified by this study could be employed as the guidelines for developing a formal and comprehensive education program for discharge planning. Accordingly, an identical work model for the discharge planner could be developed in the best interest of the clients.
