目的 觀察原發性高血壓(EH)與腎實質性高血壓(RH)患者腎上腺髓質素前體N端20肽(PAMP)水平的變化，探討PAMP的降壓機制。方法 用放免法測定EH與RH患者的血漿PAMP，用高壓液相(HPLC)法測定去甲腎上腺素(NE)、腎上腺素(E)的水平。結果 EH及RH患者PAMP、NE、E均較正常人明顯增高，PAMP、MAP與NE、E有明顯相關性。結論 EH及RH患者PAMP增高可能是機體維持自穩態的一種保護性代償機制，EH與RH代償程度有所不同。PAMP降壓作用與拮抗NE、E有關。
Objective To assess the changes of proadrenomedullin N-terminal 20 peptide (PAMP) in patients with essential hypetension (EH) and renal hypertension (RH) and explore the mechanism of PAMP in lowing blood pressure. Methods Plasma concentrations of PAMP、nor-adrenalin (NE) and adrenalin (E) in EH and RH patients were measured by radio-immunoassay(RIA) and HPLC and their associations were calculated. Results Plasma concentrations of PAMP、NE and E in patients with EH and RH were significantly higher than those of normal people. PAMP and the mean arterial pressure (MAP) had significant correlation with NE and E. Conclusions PAMP is a vasodilator peptide. The increase of plasma PAMP may be the self-compensatory mechanism by which the body maintains homeostasis. There was a difference in compensatory level between the patients with EH and RH. The action of PAMP in lowing blood pressure is due to the inhibition of NE and E.
