目的 检测肾移植患者外周血淋巴细胞亚群、单个核细胞凋亡情况,结合临床诊断及临床表现,探讨肾移植患者手术前后淋巴细胞亚群、外周血单个核细胞凋亡及免疫抑制治疗的关系。方法 收集2例移植患者术前2天,术后2天,8天,16天外周血,利用流式细胞仪进行淋巴亚群及PBMC凋亡检测。结果 与正常参考值相比,两例肾移植患者CD3+细胞百分率(61.54~73.14)%在正常范围,CD3(上标 -)CD19(上标 +)细胞百分率较高(11.14~29.06)%,CD3(上标 -)CD16+56(上标 +)细胞百分率(3.54~25.50)%较低;CD4(上标 +)细胞百分率逐渐增加(30.48~41.06)%;CD8(上标 +)细胞百分率(26.55~34.08)%则逐渐降低(31.74~27.02)%;CD4/CD8比值(0.96~1.52)逐渐升高;PBMC凋亡率术前2天分别为10.26%、8.28%,术后2天分别为14.32%、11.75%、术后8天分别为12.93%、8.42%,术后16天分别为9.13%、5.76%。结论:在免疫抑制剂与患者免疫系统的综合作用下,肾移植患者术后2天和术后8天PBMC细胞凋亡率较高;CD4/CD8比率控制在正常范围内。

English Abstract

Objective: To study the relationship among lymphocyte subsets、peripheral mononuclear cells (PBMC) apoptosis and immunosuppressant by examine PBMC apoptosis and lymphocyte subsets in kidney transplant recipients. Methods: Heparin treated peripheral bloods were collected from 2 kidney-transplanted patients, PBMC apoptosis and lymphocyte subsets were tested by flow cytometry (FCM). Results: The percentage of CD3(superscript +) cells (61.54~73.14)% was within the normal range; CD3(superscript -) CD19(superscript +) cells (11.14~29.06)% and CD4(superscript +) cells (30.48~41.06)% were higher than normal range, while CD3(superscript -)CD16+56(superscript +)(3.54~25.50)% and CD8(superscript +) cells (26.55~34.08)% were lower. The ratio of CD4+ cells and CD8+ cells became higher. The apoptosis percentage of PBMC in two patients was different, but both of rates were higher in day 2 and day 8 post-transplant, and were lower in day 2 pre-transplant and day 16 post-transplant. Conclusion: PBMC apoptosis were high in day 2 and day 8 post-transplant, and CD4(superscript +)/CD8(superscript +) ratio was improved under Cyclosporin、Prednisone and Azathioprine treatment.
