Primary Malignant Fibrous Histiocytoma of the Pleura: Report of a Case
原發性肋膜惡性纖維組織細胞瘤：病例報告
摘要
惡性纖維組織細胞瘤是成年人最常見的惡性軟組織肉瘤，以男性居多，好發於四肢及後腹腔。原發於肋膜的惡性纖維組織細胞瘤卻極少見，目前文獻上僅有七個病例被發表過。本例為七十五歲男性，在肋膜內患有巨大惡性纖維組織細胞瘤，病人接受胸壁腫瘤切除及右下肺葉楔狀切除。確立診斷主要依據病理顯微及免疫組織化學檢查。病人拒絕接受化學治療或放射線治療，然而術後追蹤八個月並無復發或轉移的跡象。

Malignant fibrous histiocytoma (MFH) is the most common soft tissue sarcoma of adulthood with a male preponderance, and it often involves the extremities and retroperitoneum. Primary malignant fibrous histiocytoma of the pleura is extremely rare. We report a 75-year-old male of huge MFH of the right pleura. A thoracotomy with chest wall resection and wedge resection of the right lower lobe (RLL) of the lung was performed. The diagnosis was established on the basis of pathologic microscopic findings and immunohistochemistry. Postoperatively, the patient refused adjuvant chemotherapy or radiotherapy. We followed up this case for 8 months without local recurrence or metastasis. Most studies recommend adjuvant chemotherapy or radiotherapy for these tumors, but its role merits further investigation.
