Excessive particulate polyethylene wear is one of the important caused for the aseptic loosening. Derivation of the socket penetration depth with the radiographic methods, is generally used for the understanding of the polyethylene wear. However, errors produced by the blurred edge of images in the analyzing process are usually neglected and underestimation of the true wear is thus occurred. Here, an improved algorithm for calculating wear with clinical radiographs is proposed. The algorithm may deterimine the wear of acetabular component of a total hip replacement accurately by examining the standardized initial and followed-up radiographs. In this study, a numerical model is developed to compute the wear of living polyethylenes. Based on samples of 108 total hip implants. Results obtained from this new model reveal that the present algorithm is significant. Mean linear wear rate of the polyethylene derived by the present model is also consistent with previous findings in medical engineering fields. Moreover, the present model may be used to compute the linear wear, the volumetric wear, the anteversion of the cup, the inclination of the cup and the direction of socked wear sufficiently.
