創傷性腦傷病患住院期間之物理治療成果與療程分析

The Functional Outcome of Hospitalized Cases with Traumatic Brain Injury Receiving Physical Therapy

Case payment system is one of the prospective payment system (PPS) likely to repleace the currentsement system for physical therapy service in Taiwan in the near future. Thus it is important to understand the time spectrum and the treatment outcome of physical therapy for each disease. The aims of this study ware to demonstrate the treatment outcome in the acute hospitalized phase in cases with traumatic brain injury and to show the time interval needed tor functional activities training. Ninety-three subjects treated in physical therapy department in a medical center were recruited from 1994 to 1998. The physical therapy records were reviewed retrospectively. There were 51 subjects had 2 or more records to show improvement in physical examination and functional activities. The results showed that conscious status was the only factor showed significant improvement after physical therapy interventions in all of the items of physical examinations (p<0.05). However, much more subjects showed improvement in the performance of functional activities. More subjects were able to roll (p<0.05), sit up and lie down (p<0.0l), transfer from chair to bed and from bed to chair (p<0.05), stand up and sit down (p<0.05) independently or in supervision after physical therapy. More subjects demonstrated got good or fair sitting (p<0.05) and standing (p<0.01) balance after treatment. On the average, the subjects needed 1 week to be trained to roll, 2 weeks to achieve the transfer ability and 1 month to ambulate independently. A home care system is warranted for those patients who were not functionally independent before discharged from hospital. Items should be recruited in the chart of physical therapy were also suggested by the author.
