摘要
腮腺脂肪瘤在臨床上相當罕見，所以通常不被列爲術前鑑別診斷之一。臨床上，電腦斷層掃瞄與核磁共振掃瞄具有診斷價值，而細針穿刺細胞學檢查則可做爲診斷參考但僞陰性高。上述檢查皆無法完全排除脂防肉瘤(liposarcoma)的可能。最好的治療方式是完整切除以確定診斷並且避免復發。本科於2008年9月經歷1名44歲男性病患，主述左上頸部一漸進性腫塊已有兩個月。理學檢查發現左側腮腺有一約3.0×2.0×3.0cm軟性腫塊。電腦斷層檢查顯示左側腮腺內有一低密度訊號腫瘤，大小約爲2.2×1.6×25cm。在腮腺脂肪瘤的臆測之下，遂安排左側淺葉腮腺切除術。病理診斷證實爲腮腺內脂防瘤。術後追蹤迄今無明顯復發。由於此病例罕見特提出報告並回顧相關文獻。

Lipoma of the parotid gland is a rare clinical entity and is seldom considered in the preoperative differential diagnosis of the parotid tumors. Clinically, the computed tomography scan (CT) and magnetic resonance imaging (MRI) offer diagnostic value. Fine needle aspiration cytology (FNAC) can also provide diagnostic reference whereas its high false-negative rate should be noted. Nevertheless, all the above examinations can not totally rule out the potential possibility of malignancy. The optimal treatment will be conducting a complete excision to confirm the diagnosis and prevent recurrence. This article presents a patient of 44-year-old man whose chief complaint was a progressive mass on his left upper neck for about 2 months. Physical examination revealed a left parotid tumor of about 3×2×3 cm in size. The CT scan showed a low density tumor (-102 Hounsfield units) in the left parotid gland. The tumor was 2.2×1.6×2.5 cm in size. Under the impression of lipoma of the left parotid gland, left superficial parotidectomy was performed. Pathologic study confirmed the diagnosis of intraparotid lipoma. The patient was followed-up for 1 year without recurrence. We present this rare case and review the related literatures.
