Post-radiation sarcoma (PRS) is uncommon and most of them were found years after the initial radiotherapy. Treatment is difficult due to its aggressive behavior and the outcome is poor. We report a case of PRS developed 10 years after the initial radiotherapy for nasal NK cell lymphoma. Orbital extension and skull base involvement is apparent and a combined operation was performed.
