眼窩峰組織炎-32例病例臨床分析

Review of 32 Cases of Orbital Cellulitis

摘要


眼窩峰窩組織炎是眼科常見的急症,其症狀嚴重者可能影響視力,甚至致命。本文收集本院最近5年之中,眼窩峰窩組織炎患者共32例,年齡尚23天至83歲,男性10例,女性22例,從症狀產生至接受治療平約天數4.3天,平約住院天數7.8天,只有15例(47%)可追蹤到發病原因,其餘7例原因不明。18例接受局部細菌培養,致病細菌有16例可被確認,其中以金黃色葡萄球菌佔7例(21%)最多。32例中有12例接受手術治療,包括眼窩膿瘍引流8例,鼻竇手術7例,1例接受眼球內容物剜除術(Evisceration)。併發症只有2例,1例復發,1例中心視網膜動脈阻塞導致失明。顯示經廣效抗生素及適當外科引流治療,眼窩峰窩組織炎可以有很好的預後。

Orbital cellulitis is an ocular emergency that not only threatens vision but also may be fetal. We analysed retrospectively a total of 32 patients with orbital cellulitis over a period of five years in the 804 Army General Hospital in Tao-Yuan. The age ranged from 23 days to 83 years. Ten were male and 22 were female patients. There was a 4.3 days delay in average between the first symptom and hospitalization, with a mean hospital stay of 7.8 days. Fifteen out of 32 patients had traceable predisposing factors. Eighteen patients received wound cluture, causative organisms could be identified in sixteen of the patients. Seven patients had Staph. aureus infection. Twelve patients received surgical intervention, including drainage of orbital abscess in eight patients, drainage of sinuses in seven, and evisceration in one. There were only two complications, one had recurrent orbital cellulitis due to under treatment of antibiotics, one developed central retinal artery occlusion and remained blind after treatment. This study showed appropriate antibiotics treatment and timely surgical intervention may lead to a good prognosis and may avoid serious complications.
