Fat Embolization Retinopathy: A Case Report

摘要

脂肪栓塞性視網膜病變是一種相當罕見的繼發性疾病，大多發生在長骨骨折之後，其中又以股骨及骨盆骨骨折最為常見。其最主要的病徵為雙側眼底散佈性的棉絮狀滲液及視網膜內點狀出血。我們提出一位26歲的年輕男性，在股骨骨折之後數天，突然發生嚴重頭痛及雙眼視力模糊的情形，經過檢查後發現，眼底有典型的散佈性棉絮狀滲液及視網膜內點狀出血。然而，三天之後，病人的視力自動回復正常，且視網膜的病變，在兩個月後的追蹤檢查也發現已經消失。脂肪栓塞性視網膜病變，一般在臨床上都有良好的預後，只需要正確的診斷及保守療法。這篇文章的目的即在於增進我們對此種疾病的了解，並試着與其它類似疾病作鑑別診斷。

Fat embolization retinopathy is secondary to the fat embolism syndrome (FES) and appears as bilateral cotton wool exudates and intraretinal hemorrhages. It's a rather rare delayed complication of lower extremities and the pelvis fracture. The most likely factors producing the syndrome are fractures of lower extremities and the pelvis. We describe a 26 year-old male patient who suffered from sudden attack of bilateral decreased vision and headache several days after femoral shaft fracture. Fundus examination disclosed typical bilateral cotton wool exudates and dots of intraretinal hemorrhage. Spontaneous recovery of visual acuity was observed 3 days later and fundus returned to normal within 2 months. Fat embolization retinopathy usually has good prognosis and only conservative treatment is necessary. Differential diagnosis with some traumatic retinopathies which have the similar retinal findings is discussed.
